摘　　要：目的探讨奥曲肽联合埃索美拉唑对肝硬化食道胃底静脉曲张破裂出血患者血浆内毒素和一氧化氮(NO)水平的影响及疗效观察。方法 72例肝硬化食道胃底静脉曲张破裂出血患者随机分为实验组与对照组,分别予以奥曲肽联合埃索美拉唑和单纯奥曲肽治疗,连用7 d。结果治疗7 d后,两组血浆内毒素和NO水平均明显下降(P<0.05或P<0.01),且实验组下降值较对照组更明显(P<0.05)。实验组治疗后临床疗效明显高于对照组(χ2=5.67,P<0.05)。实验组和对照组治疗期间不良反应5例和3例,症状较轻,两组比较无明显统计学差异(χ2=0.14,P>0.05)。结论奥曲肽联合埃索美拉唑治疗肝硬化食管胃底静脉曲张破裂出血的疗效确切,安全性较好,作用与降低血浆内毒素和NO水平密切相关。Objective To discuss the influence and curative effect observation of octreotide combined with esomepra- zole on toxin in plasma and nitric oxide（NO） levels of liver cirrhosis with esophageal variceal bleeding patients. Meth- ods Seventy-two cases of nitric oxide levels of liver cirrhosis with esophageal variceal bleeding patients divided into experimental group and control group were given octreotide combined with esomeprazole and octreotide only respec- tively for 7 days. Results After 7 days＇ medical treatment, the toxin in plasma and NO levels of patients in two groups declined obviously than before （P〈0.05 or P〈0.01 ）, and the declining rate in experimental group was much higher than that in control group （P〈0.05）. The total clinical efficiency of experimental group was much higher than that in control group （X2 =5.67,P〈0.05）. 5 and 3 cases of untoward effect appeared in experimental group and control group during the medical treatment with light symptom. After comparing the DAR occurrence rates, no obvious statis- tical differences appeared（x2=0.14,P〉0.05）. Conclusion Octreotide combined with esomeprazole has reliable curative effect on liver cirrhosis with esophageal variceal bleeding with high security, whose mechanism of action has close ef- fect on reducing the toxin in plasma NO levels, and improving the hemodynamic disturbance of patients.
