德宏州1608例女性HPV感染基因分型情况的调查

摘　　要：目的 了解德宏地区女性宫颈感染高危人乳头瘤病毒（HPV）基因型的分布情况.方法 采用PCR和分子杂交技术对1608例就诊女性进行HPV基因分型检测.结果 1608例女性中HPV阳性651例,感染率为40.48％;其中HPV低危型感染71例,高危型感染580例,排在前5位的HPV高危基因型依次是HPV16（23.35％）、HPV58（15.36％）、HPV52（9.37％）、HPV33 （7.22％）和HPV18（4.61％）.HPV高危基因型单一感染462例（70.97％）,二重感染95例（14.59％）,三重感染22例（3.38％）.在651例感染者中,HPV16单一感染152例,HPV16合并其他基因型双重和三重感染67例.感染年龄高峰在30～50岁.结论 1608例筛查女性中,HPV感染率较高,达40.49％,以HPV16基因型为主.HPV分型检测对高危年龄段女性健康检查、预测病变进展、评估预后、指导治疗有重要价值.Objective To understand the genotype distribution of high-risk human papilloma virus （HPV） by cervical infection in Dehong area.Methods The HPV genotype test was performed in 1608 female cases by PCR and molecular hybridization.Results 651 cases out of 1608 cases were positive in HPV,accounted for 40.48%.Among them,the num- ber of low-risk and high-risk HPV infections was 71 cases and 580 cases respectively.The high-risk of HPV geno- types in top 5 places were HPV16 （23.35%）,HPV58 （15.36%）,HPV52 （9.37%）,HPV33 （7.22%）,and HPV18 （4.61%） in turn.Among high-risk HPV genotypes,the number of single infection,double infection,and triple infection was 462 cases, 95 cases,and 22 cases,occounted for 70.97%,14.59%,and 3.38% respectively.There were 152 cases of HPV16 single infection cases,and 67 cases double and triple infection cases on the combination of HPV16 and other genotypes.The peak of infection ages ranged from 30 to 50 years old,Conclusion Among 1608 cases of female cases,the HPV infec- tion rate is high and reaches 40.49%.The HPV16 is the principal one compared with other HPV genotypes.HPV geno- type test is of great importance in physical examination,prediction of disease progression,prognosis evaluation,and treat- ment guidance in high-risk female in certain age group.
