固定临时冠桥在牙齿重度磨耗修复中的临床应用

摘　　要：目的研究固定临时冠桥在牙齿重度磨耗修复治疗中的作用,探讨是否能够成为口颌系统恢复咬合高度、重建咬合平衡的一种方法。方法选择重建咬合后可以行固定义齿修复的重度磨耗患者8例,以固定临时冠桥修复牙齿重度磨耗的垂直距离(重建咬合),分别在重建咬合后1、2、3个月随访观察重建后的效果包括颞颌关节的适应性、面部软组织形态变化、咀嚼功能的重建等。结果 8例中有7例在定期调牙合、磨改后,开始适应重建后的垂直距离,均达到预期修复目的。剩余1例由于咬合重建之前有严重的磨牙症,致前牙区固定义齿有部分咬合面崩瓷现象。结论在对重度磨耗病例行永久性固定义齿修复之前,利用固定临时冠桥重建咬合,不仅有效地恢复了组织原有的功能,而且解除了磨耗引起的过敏疼痛等症状给患者带来的痛苦,可能成为口颌系统恢复咬合高度、重建咬合平衡的一种有效方法。Objective To study the effects of fixed temporary crown and bridge on the reparation of severe tooth wear, to discuss the feasibility of the restoration of occlusal height in gnathostomatic system and the reconstruction of occlusal balance with this method. Methods Eight patients with severe tooth wear who would receive the reparation by fixed denture after the occlusal reconstruction were selected. The fixed temporary crown and bridge was used to repair the vertical distance of severe worn tooth（ occlusal reconstruction）. The adaptability of temporomandibular joint, the morphologic changes of facial soft tissues, and the reconstruction of masticatory function were followed-up in 1,2, and 3 months after the occlusal reconstruction. Results Among the 8 cases ,7 ones began to adapt to the reconstructed vertical distance after regular adjusting occlusion and occlusal grinding, and all achieved the expected reparation purposes. Only one case had partial fractured porcelain in the fixed denture in the anterior teeth region because it had severe bruxism before the reconstruction. Conclusion Before the reparation by permanently fixed denture for patients with severe tooth wear, the use of fixed temporary crown and bridge in the occlusal reconstruction can not only effectively restore the original function ,but also relieve hypersensitiveness and pain caused by wear. This method is an effective way to the restoration of occlusal height in gnathostomatic system and the reconstruction of occlusal balance.
