匹伐他汀与阿托伐他汀对缺血性脑卒中患者血脂调控的临床对比研究

摘　　要：目的评价匹伐他汀与阿托伐他汀在缺血性脑卒中患者血脂调控的疗效，对比改善缺血性脑卒中患者颈动脉粥样硬化的差异。方法选择急性脑梗死患者54例，随机分为2组，匹伐他汀组30例，接受匹伐他汀1～2mg／晚治疗，其中前循环梗死患者20例，后循环梗死患者7例，分水岭梗死患者3例；阿托伐他汀组24例，接受阿托伐他汀10mg／晚治疗，其中前循环梗死患者12例，后循环梗死患者12例。治疗期共24周。结果匹伐他汀组和阿托伐他汀组治疗后24周LDL-c显著下降，差异有统计学意义（P〈0．01）。匹伐他汀组治疗后24周右颈总动脉内膜中层厚度有所减少，但差异无统计学意义（P〉0．05）。结论匹伐他汀降低LDL-c的作用和治疗颈动脉粥样硬化的远期作用可能与阿托伐他汀相当，不良反应较轻微，可用于高胆固醇血症和动脉粥样硬化的治疗。Objective To assess the therapeutic effect of pitavastatin and atorvastatin on regulation of lipid and carotid artery atherosclerosis in ischemic stroke patients. Methods Fifty-four acute cerebral infarction （ACI） patients were randomly divided into pitavastatin group （n= 30） and ator- vastatin group （n=24）. Of the patients in pitavastatin group who received 1-2 mg pitavastatin at night,20 were diagnosed with anterior circulation infarction,7 were diagnosed with posterior cir- culation infarction,and 3 were diagnosed with watershed infarction. Of the patients in atorvastatin group who were given 10 mg atorvastatin at night, 12 were diagnosed with anterior circulation in farction and 12 were diagnosed with posterior circulation infarction. The patients were treated for 24 weeks. Results The serum LDL-C level was significantly lower in pitavastatin group and ator- vastatin group 24 weeks after treatment than before treatment （P〈0.01）. Although the IMT of the right common carotid artery was thinner in pitavastatin group than in atorvastatin group 24 weeks after treatment, no significant difference was found in it between the two groups （P 0.05）. Conclusion Pitavastatin and atorvastatin have a similar inhibitory effect on serum LDL-C level and carotid artery atherosclerosis. However, the adverse reaction of pitavastatin is milder than that of atorvastatin,showing that pitavastatin can be used in treatment of hypercholesterol- emia and atherosclerosis.
