放疗联合热疗治疗盆腔恶性肿瘤术后局部复发临床观察

摘　　要：目的：探讨放疗联合热疗治疗盆腔恶性冲瘤术后局部复发患者的近期疗效和不良反应。方法：对2008年1月～2012年10月收治56例盆腔恶性肿瘤术后局部复发患者按随机分组原则分为单纯放疗组和放疗加体外热疗组。单纯放疗组（28例）：采用直线加速器常规分割照射，剂量分割：2．0Gy／次，1次／d，5次／w。累计总剂量60～70Gy。放疗加体外热疗组（28例）：热疗采用HG-2000型体外高频热疗仪，放疗期间给予热疗2次／w，热疗间隔时间≥72h，配合至放疗结束。每次热疗均在放疗之前1h内进行，热疗温度保持在40～43℃，持续加热时间维持至少40min以上。结果：单纯放疗组完全缓解率25．0％（7／28），放疗加体外热疗组完全缓解率53．6％（15／28），单纯放疗组总有效率（完全缓解＋部分缓解）53．6％（15／28），放疗加体外热疗组总有效率85．7％（24／28），两组比较差异有统计学意义（P〈0．05）。两组近期不良皮肤反应、直肠反应、膀胱反应、骨髓抑制单放组分别为5例、11例、5例、3例，放疗加体外热疗组为3例、10例、4例、3例，两组比较差异无统计学意义（P〉0．05）。结论：放疗加热疗组完全缓解率、总有效率明显提高，放疗加体外热疗治疗盆腔恶性肿瘤术后局部复发在肿瘤消退方面的效果明显优于单纯放射治疗，在放疗剂量不降低的情况下近期不良反应并无增加。Objective： To explore the short- term effect and adverse reactions of radiotherapy combined with hyperthermia in patients with local recurrence of pelvic malignancies. Methods： In January 2008 to October 2012, 56 cases of pelvic malignant tumor local recurrence were randomly treated by radiotherapy and radiothera- py plus external high frequency hyperthermia. Radiotherapy alofle group （28 cases） ： the conventional fractionat- ed radiotherapy, split dose： 2.0 Gy/d, 5 times/week, total dose of 60-- 70 Gy. Heat the HG- 2000 type of ex- tracorporeal high - frequency hyperthermia instrument, radiation to heat during 2 times/w, heat treatment time interval ≥ 72 h, with until the end of radiotherapy. Each hypertherrnia were performed within 1 hours prior to radiotherapy, hyperthermia temperature maintained at 40- 43 ℃, continuous heating time maintained for at least 40 minutes. Results： Radiotherapy alone group complete relief rate was 25 % （7/28）, Radiotherapy plus ex-ternal heat therapy group complete relief rate was 57.1% （15/28）, radiotherapy alone group, the total efficien- cy（full mitigation ＋ part of the mitigation）of 53.6 % （15/28）, Radiotherapy plus external heat therapy group the total efficiency of 85.7 % （24/28）; the difference between the two groups had statistical significance （P 〈 0.05）. About adverse skin reactions, rectum reaction, bladder reaction, bone marrow suppression, radiotherapy alone group were 5 cases, 11 cases, 5 cases, 3 cases, respectively, radiotherapy plus hyperthermia group respec- tively were 3 cases, 10 cases, 4 cases, 3 cases. The difference was not statistically significant（P〈0.05）. Con- clusion： Radiotherapy plus external heat therapy group complete remission rate the total efficiency is improved obviously, radiotherapy with high frequency hyperthermia in vitro for postoperative local recurrence of pelvic malignancies tumor regression was significantly superior to radiotherapy alone, In the case of radiation dose not reduce adver
