摘 要:目的探讨成人部分性癫痫患者的睡眠结构与听觉事件相关电位(AERPS)的特征及相关性。方法对我院2010年7月—2013年1月初诊断的45例成人部分性癫痫患者(癫痫组)和40例健康对照人群(对照组)行睡眠脑电图监测和AERPS检测,分析其睡眠参数和AERPS各波潜伏期及波幅。结果与对照组比较,癫痫组总睡眠时间减少、睡眠效率下降、非快速眼动睡眠(NREM)1、2期占总睡眠时间的百分比增加、NREM慢波睡眠期(3+4期)占总睡眠时间的百分比下降、清醒期占总睡眠时间的百分比增加,差异均有统计学意义(P<0.05);而睡眠潜伏期、快速眼动睡眠(REM)潜伏期、REM期占总睡眠时间的百分比和觉醒次数两组间差异均无统计学意义(P>0.05)。与对照组比较,癫痫组AERPS的P300潜伏期延长,差异有统计学意义(P<0.05);而P300波幅,N100、P200和N200的波幅及潜伏期两组间差异均无统计学意义(P>0.05)。癫痫组P300潜伏期与睡眠效率、NREM慢波睡眠期(3+4期)占总睡眠时间的百分比均呈负相关(r值分别为-0.43、-0.48,P<0.05),与记录的其余睡眠参数无明显相关关系。结论成年部分性癫痫患者有睡眠结构异常,AERPS的P300潜伏期延长,认知功能受损;睡眠参数异常与P300潜伏期有一定的相关性。Objective To explore the characteristics of sleep architecture and auditory event - related potentials (AERPS) of adult patients with partial epilepsy and the relationship between sleep and AERPS. Methods A total of 45 adult patients with partial epilepsy treated in our hospital from July 2010 to January 2013 were included as the test group, and 40 healthy adults were recruited as the control group. The sleep architectures of all participants were monitored with sleep EEG, and AERPS were also measured. The sleep parameters and the latencies and amplitudes of all waves of AERPS were analyzed. Results Compared with the parameters in the control group, the total sleep duration in the test group was shorter (P 〈 0. 05 ) ; sleep efficiency was lower (P 〈0.05) ; the percentages of non -rapid eye movement sleep (NREM) stage 1, NREM stage 2, and wakefulness were higher (P 〈 0. 05) ; while the percentage of slow - wave sleep ( NREM stage 3 and NREM stage 4) was lower (P 〈 0.05) . There were no significant differences on sleep latency, REM latency, the percentage of REM sleep, and arousal index between the two groups. The test group had a significant longer P300 latency than the control group did ( P 〈 0. 05), and no significant differences were noted on the amplitudes and latencies of N100, P200 and N200 between the two groups. In the test group, the tB00 latency was negatively correlated with sleep efficiency and the percentage of slow - wave sleep ( r = - 0.43, - 0. 48 ; P 〈 0. 05 ), no significant correlations was found between other study variables. Conclusion The sleep architecture of adult patients with partial epilepsy is abnormal. The patients have a prolonged P300 latency of AERPS, and their cognitive func- tion is impaired. The abnormal sleep architecture may be related to P300 latency.
