摘 要:目的利用扩散张量成像(DTI)及静息态功能磁共振(rs-fMRI)区域局部一致性(ReHo)技术,观测复发-缓解型多发性硬化(RRMS)患者海马区表观扩散系数(ADC)、各向异性分数(FA)和ReHo的变化,推测其海马区结构、功能的改变及两者可能存在的相关性。方法 17例RRMS患者与17名年龄、性别相匹配的健康志愿者接受rs-fMRI和DTI扫描,分别选择左、右侧海马为感兴趣,对两组间双侧海马ReHo进行统计参数图谱分析及左、右侧海马区域ADC、FA的两样本t检验分析,组内单侧海马ADC、FA及ReHo相互进行Pearson相关性分析。结果与健康对照组比较,RRMS患者左侧海马ADC(t=3.147,P=0.004)及FA(t=3.544,P=0.001)均存在显著差异;RRMS患者左侧海马头部、尾部ReHo减低(P<0.05,经AlphaSim校正,体素>10)。健康对照组及RRMS患者组海马ADC值与FA值存在明显负性相关。患者组左侧海马ADC值(r=-0.544,P=0.036)、FA值(r=0.508,P=0.037)与左海马尾部ReHo值分别存在负性相关和正性相关。结论 RRMS患者存在海马区结构及功能异常,且这种改变存在偏侧化现象。海马功能的异常与其自身结构的改变密切相关,可能是认知障碍形成的原因之一。Objective To evaluate the hippocampus abnormality in the RRMS subjects by using diffusion tensor ima- ging(DTI) and ReHo based on altered ADC, fractional anisotropy (FA) and ReHo value. Methods seventeen patients with RRMS and 17 gender and age-matched healthy controls(HCs)underwent rs-fMR[ and DTI. Bilateral hippocampus were selected as ROIs to measure ReHo, ADC and FA values which were analyzed by statistic parameter spectrum and two- sample test respectively. ADC, FA values and ReHo values of each ROI were analyzed using Pearson correlation analysis. Results Compared with normal controls, left hippocampus of RRMS patients showed significantly increased ADC ( t = 3. 147, P = 0. 004 ) and decreased FA ( t = 3. 544,P = 0. 001 ). ReHo of brain function of resting-state in the cranial and cau- dal parts of left hippocampus was decreased ( P 〈 0.05, AlphaSim correction, voxel 〉 10). ADC values had significantly negative correlated with FA values in each lateral hippocampus in HCs and RRMS patients. In the caudal part of left hippo- campus, ADC values had negative correlated with ReHo values (r = -0. 544,P = 0. 036) in RRMS patients and FA values had positive correlated with ReHo values (r = 0. 508 ,P = 0. 037 ). Conclusion RRMS patients showed an abnormality of brain function and structure of hippoeampus, and revealed lateralization. There was significant relation between function and structure in hippocampus,which was suggested being one of major cause on cognitive impairment.
