颌下区原发性肿瘤的CT诊断

摘　　要：目的探讨颌下区原发性肿瘤的CT特征。方法回顾性分析经手术病理证实的38例颌下区原发性肿瘤的CT表现。结果颌下腺混合瘤18例,其中7例为稍低密度,3例为稍高密度,1例为等密度,7例密度不均匀。颌下腺恶性肿瘤10例,4例密度均匀,6例密度不均匀;肿瘤与颌下腺分界不清或颌下腺显示不清。蔓状血管瘤3例,表现为密度不均匀,可见钙化,2例与颌下腺分界清楚。淋巴瘤7例,3例为双侧,4例为单侧,1例与颌下腺分界清楚,4例与颌下腺分界不清。良恶性肿瘤在患者年龄、病灶边界是否清晰、是否均匀强化、有无肿大淋巴结及有无坏死囊变差异具有统计学意义,而性别及病灶内有无钙化差异无统计学意义。结论 CT对颌下区原发性肿瘤的定位及定性诊断具有重要意义。相关CT征象对颌下区肿瘤的良恶性鉴别有重要意义,但对于来源于颌下腺的恶性肿瘤难以确定其组织学类型。Objective To explore CT manifestations of primary tumors in submandibular region. Methods The CT findings of 38 patients with primary tumors in submandibular region proved by operation and patho]ogy were retrospectively studied. Results Of 38 patients, 18 were pleomorphic adenomas,7 lesions showed low attenuation compared with normal glandular tissue, 3 lesions were high attenuation, 1 lesions were iso-attenuated and 7 lesions were heterogeneous in density on plain CT. l0 of 38 were malignant neoplasm originated from submandibular glands, 4 lesions were iso-attenuated and 6 lesions showed heterogeneous. There were ill defined borders between the lesions and the submandibular glands. All the 3 cases of hemangioma racemosums had sharp margins, 2 cases showed heterogeneous in density and calcifications, 1 lesion was low density. Among 7 cases of lymphoma,3 were bilateral,4 were unilateral. 1 lesion had sharp margin, and 4 lesions had ill defined border with the suhmandibular glands. There was statistical significance in clear borders, even enhance- ment, enlarged lymph nodes and cystic change between the benign and maliganent tumors of submandihular region. Con- clusion CT has important value in identifying the location and property of the primary tumors in suhmandibular region. It is difficult to categorize the histological types of the malignant tumors originated from suhmandibular gland.
