球囊辅助技术在颅内宽颈动脉瘤栓塞治疗中的应用

摘　　要：目的探讨和总结球囊辅助弹簧圈血管内治疗颅内宽颈动脉瘤的技术及疗效。方法回顾性分析采用球囊辅助可脱弹簧圈治疗30例（33枚）颅内宽颈动脉瘤患者的临床资料。结果球囊结合弹簧圈完成栓塞33枚动脉瘤，动脉瘤100％栓塞27枚，90％栓塞2枚，80％栓塞4枚。1例动脉瘤在栓塞前破裂，采用球囊暂时封闭瘤颈阻止出血，然后进行栓塞，挽救了患者的生命。1例动脉瘤栓塞即将结束时破裂，采用球囊封闭瘤颈数分钟后出血停止。球囊辅助包括7枚微小动脉瘤。无死亡。随访3—24个月30例患者无再出血及脑梗死。结论球囊结合弹簧圈栓塞颅内宽颈动脉瘤有较好的疗效，在使用中应注意球囊使用的特点。Objective To explore the efficacy and its curative effects of endovascular embolization with balloon assited technique in treatment of wide necked intracranial aneurysms. Methods Thirty three wide necked intracranial aneurysms in 30 patients were slected for endovascularly embolized treatment with the balloon assisted detachable coil technique. Re- suits Of 33 aneurysms, 27 were completely occluded, 2 occluded by 90% , 4 occluded by 80%. There were no rebleed- ing and thromhoembolic events in 22 patients followed up from 3 to 24 months. Conclusion The balloon assisted coil technique was a safe and effective treatment for wide necked aneurysms. Anti platelet drug induced risks should be paid at- tention to in the patients with aneurysms.
