摘  要:目的:探讨细胞周期调控因子表达在大肠肿瘤发生发展中的作用及其对预后的影响。方法:采用免疫组化染色对 113例大肠癌、 14例大肠腺瘤、 11例腺瘤癌变组织中 cyclin D1、 p21WAF1/CIP1、 p53和 PCNA进行了检测。结果:大肠癌组织 cyclin D1和 p53阳性表达率分别为 54.87%和 56.64%,均显著高于正常大肠组织 (P<0.05),高分化腺癌P21蛋白阳性表达率)69.70% ,低分化腺癌为40.91%,两者间相差显著(P<0.05 ),大肠癌、腺瘤癌变的PCNA LI 较腺瘤组显著增高(P<0.01)。cyclin D1、P53、PCNA 过度表达者,其5年生存率均显著降低。结论: cyclin D1、P53、 过度表达与P21失活可加速细胞周期转化,促进大肠癌的发生, P21蛋白过度表达可能与大肠癌分化有关, cyclin D1、P53和PCNA 过度表达是大肠癌预后不良的危险因素。Objective: The current study was designed to explore the role of expression of cell cycle regulators in colorectal tumor, and their influence on prognosis. Methods: The expressions of cyclin D1, p21,p53 and proliferating cell nuclear antigen (PCNA) in 113 cases of carcinomas, 14 cases of adenomas and 11 cases of malignant change in adenomas were detected by immunohistochemistry. Results: The positive rates of cyclin D1 and p53 of colorectal carcinomas were 58.87% and 56.64% , respectively, which were significantly higher than that of normal mucosa(P< 0.05). The positive rates of p21 protein in well and poorly differentiated adenocarcinomas were 69.70% and 40.91% , respectively, there were significant difference between them(P< 0.05). The PCNA laleling index (PCNA LI) of carcinomas and malignant areas in adenoma were significantly higher than that of adenomas( P< 0.01) . The 5 year survival rate was significantly lower in the patients with cyclin D1, p53 or PCNA strongly positive tumors than in those with negative tumors (P< 0.05). Conclusion: The overexpression of cyclin D1, p53 protein and inactivation in p21 gene mutation might promote cell cycle progression and carcinogenesis.The overexpression of p21 protein can inhibit the proliferation and promote the differentiation of carcinoma cells. The overexpression of cyclin D1, p53 and PCNA are the risk factors for the poor prognosis of colorectal cancer patient.
