肝癌及癌旁组织p21蛋白表达与其病理学指征的相关性

摘　　要：目的： 研究肝细胞癌（ hepatocellular carcinoma, HCC）及癌旁组织 p21蛋白表达的意义。方法：应用免疫组化方法结合计算机图像分析技术测定 47例肝癌手术标本 p21蛋白的表达水平，并与临床病理学指标进行对比。 结果：癌旁组织 p21蛋白表达阳性率及表达水平均高于癌组织（ P<0.01),癌旁组织 p21蛋白表达水平在侵袭转移性HCC高于非侵袭无转移性HCC(P<0.01).并且癌旁组织P21蛋白表达水平高于癌组织的HCC,其侵袭转移等指标亦高于癌旁组织P21蛋白表达水平低于癌组织的病例(P<0.01).结论:肝癌旁组织P21蛋白的表达在评估其侵袭转移潜能方面可能是一个较癌组织本身更为有用的指标。Objective： The current study was designed to investigate the significance of expression of p21 protein in tumor- adjacent tissues of the patients with hepatocellular carcinoma（ HCC） . Methods： The expression level of p21 protein in surgical samples of 47 patients with HCC was determined by using immunohistochemical method in combination with computer- imaging analytic technique. Some clinical pathological indexes were also analyzed. Results： The p21 expression levels in tumor- adjacent tissues were higher than that in tumor tissues in most of the cases. The numbers of invasion and intrahepatic metastasis were higher in the case with higher p21 protein levels in tumor- adjacent tissues than that in the case with lower p21 protein level in tumor- adjacent tissues(P< 0.01). The tumor-adjacent tissues of HCC with invasion and intrahepatic metastasis had a significantly higher p21 expression level than those without invasion and intrahepatic metastasis(P< 0.01） . Conclusion： For evaluating the invasive and metastatic potential of HCC,the expression of p21 in tumor- adjacent tissues may be a more useful indicator than those of in tumor tissues themselves.
