摘  要:目的:分析癌基因 mdm- 2和抑癌基因 p53在胃癌及其转移灶中的变异及相互关系,探讨胃癌及其转移的分子机制。 方法:采用 DC- PCR、 PCR- SSCP及 DNA测序技术检测 32例胃癌及其转移灶中的 mdm- 2扩增和 p53突变。结果: mdm- 2在转移淋巴结中的扩增频率( 12/21, 57.1%)高于胃原发癌灶( 12/32, 37.5%), 3例淋巴微转移灶中出现 mdm- 2的扩增。在 7例肠型胃癌的原发癌或转移癌灶中同时检测出 mdm- 2扩增和 p53突变。肝转移癌灶中 p53突变位点和突变方式与胃原发癌之间有差异。结论: mdm- 2扩增可能与胃癌细胞的淋巴高转移潜能有关。 p53突变和 mdm- 2扩增多并存于肠型胃癌中。在胃原发癌和肝转移癌中癌细胞的基因改变存在着异质性。Objective: The current study was designed to reveal the correlation of alterations of oncogene mdm- 2 and tumor suppressor gene p53 with biological behaviors of gastric cancer( GC) and metastasis. Methods: The amplification of mdm- 2 and the point mutation of p53 in 32 specimens from patients with GC were detected by using DC- PCR, PCR- SSCP and DNA sequencing technique. Results: The frequency of mdm- 2 gene amplification was higher in metastasis lymphnodus (57.1% ) than that in primary carcinoma foci(37.5% ). The mdm- 2 amplification was detected in 3 specimens of lymph micro- metastasis foci. Both mdm- 2 amplification and p53 mutation appeared in primary carcinoma foci or metastasis lymphnodus of 7 case of GC. There were some differences of the location and mode of p53 point mutation in the primary carcinoma and liver metastasis foci. Conclusion: The amplification of mdm- 2 gene is correlated well with high potential lymph metastasis of the GC cells. Both p53 mutation and mdm- 2 amplification may occur in the intestinal tape GC. There is the heterogenity of p53 point mutation of the cancer cells in the stomach primary carcinoma and metastasis foci.
