摘　　要：目的：探讨 c- erbB- 2基因在结直肠癌中的表达及其对预后的影响。方法：利用免疫组化 LSAB法检测 173例结直肠癌组织中 c- erbB- 2基因表达状况，结合临床病理及长期随访资料进行分析。结果： c- erbB- 2基因主要为胞浆内及细胞膜表达，癌组织阳性率 52％。 c- erbB- 2基因表达与肿瘤浸润深度、淋巴结转移及 Dukes′分期有关，而与性别、年龄、肿瘤部位、肿瘤大小、大体类型及组织学类型无关。 c- erbB- 2阳性组术后局部复发率、远处转移率高于阴性组。 c- erbB- 2表达阴性组生存率最高，弱阳性组次之，强阳性组生存率最低。 Cox回归多因素分析提示 c- erbB- 2是死亡的危险因素，即表达水平越高，预后越差。结论： c- erbB- 2表达水平是结直肠癌高度恶性的生物学指标，可作为判断结直肠癌预后的独立因素。Objective： The current study was designed to investigate c- erbB- 2 gene expression in colorectal cancer and its influence on prognosis of the patient. Methods： We detected c- erbB- 2 protein in 173 colorectal carcinoma samples with immunohistochemical LSAB(Labelled Streptavidin Biotin) method and performed a retrospective analysis combined with data of clinicopathology and long- term follow- up for the patient. Results： The majority of c- erbB- 2 protein staining was found in cytoplasm and membrane cell with a positive rate of 52％ in tumor tissue. Expression of c- erbB- 2 gene was significantly correlated with the depth of tumour invasion, lymph node metastasis and Dukes staging of disease. There was no significant correlation between c- erbB- 2 expression and sex, age, tumor location, tumor size, gross type or histopathological type of tumor. Patients with positive c- erbB- 2 had a higher rate of recurrence and metastasis than those with negative c- erbB- 2. The 1- year, 3- year, 5- year and 10- year survival rate of patients with negative c- erbB- 2 expression was 98.7％ , 92.4％ , 87.1％ and 84.1％ , respectively, which was the best survival, and that of the patients with weak positive c- erbB- 2 expression was 94.5％ , 76.1％ , 72.3％ and 68.3％ , respectively, but that of the patients with strong positive c- erbB- 2 expression was only 91.0％ , 43.5％ , 36.8％ and 23.4％ , respectively, which was the poorest survival. The difference of survival rate among three groups of patients was statistically significant. Multivariate survival analysis was performed by Cox regression model and showed that c- erbB- 2 expression level was a risk prognostic factor for colorectal cancer, which indicated that the higher expression level of c- erbB- 2 gene is, the poorer prognosis will be. Conclusion： The expression level of c- erbB- 2 protein may be a marker for malignant biological behavior and an independent prognostic factor of colorectal carcinoma.
