子宫动脉栓塞术治疗妇产科出血性疾病的远期疗效分析

摘　　要：目的探讨子宫动脉栓塞术治疗妇产科出血性疾病的远期疗效。方法选择2004年12月至2006年12月广西北海市人民医院收治的13例患妇产科出血性疾病的患者行双侧子宫动脉栓塞术，记录手术情况、术后定期随诊，记录患者月经情况、生育情况、B超结果、子宫及子宫肌瘤的体积变化情况。结果随防时间15～96个月，平均（52．46±24．33）个月。13例患者术后能迅速止血；术后1年及2年子宫体积缩小率分别为56％及68％，与术前相比差异有统计学意义（F=7．660，P=0．02）；术后1年及2年肌瘤体积缩小率分别为76％及75％，与术前相比差异有统计学意义（F=4．653，P=0．016）；术后6个月经量恢复正常者占77％（10／13）；术后足月分娩3例，人工流产4人次。结论子宫动脉栓塞术治疗妇产科出血性疾病具有微创、安全、有效的特点，近期疗效确切，远期疗效良好，对年轻患者而言是保留子宫及生育功能的最佳治疗方法之一。Objectives To assess the long-term clinical effects of uterine arterial embolization in treating obstetric-gynecologic hemorrhagic diseases. Methods 13 patients with hemorrhagic diseases from December 2004 to December 2006 were performed with uterine arterial embolization in Guangxi Beihai people＇s Hospital. The process of procedure, data of follow-up in postoperation period, status of menstruation, status of fertility, data of ultrasound, the transformation tendency of uterus and uterine myoma, were all recorded in our study. Results All patients were followed up 15 -96 months in postoperation period. The complication of hemorrhage in 13 patients were stopped soon after the procedure. The volume of uterine had decreased 56% in 1 year as well as 68% in 2 years, respectively. And volume of uternine myoma had decreased 76% in 1 year as well as 75% in 2 years, which were statistically significant in both index. Menstruation has been recovered in 77% （10/13） of patients in six months after operation. 3 cases were found term delivery, and 4 cases were found artifcial abortion operation. Conclusions Uterine arterial embolization is effective, minimally invasive and safe in in treating obstetric-gynecologic hemorrhagic diseases. It is the one of the best therapies to preserve uterus and fertility for young women.
