豫北社区不同诊断年龄食管癌患者生存质量调查

摘　　要：目的了解豫北社区不同诊断年龄食管癌患者生存质量（QOL）状况，为食管癌患者实施个性化护理提供理论依据。方法采用便利取样法选取豫北社会951例食管癌患者进行问卷调查和访谈。调查问卷包括一般资料调查表、欧洲癌症研究与治疗组织研制的生存质量核心量表第3版（EORTCQLQ—c30v3．0）、食管癌补充模式量表（QLQOES-18）。将患者按诊断年龄分为：青年组（≤44岁）、中年组（45～59岁）、年轻老年组（60—74岁）、老年组（≥75岁）。结果青年组患者与中年组和年轻老年组患者在食欲下降方面的差异有统计学意义（P〈0．05）；年轻老年组和老年组患者在生理功能方面与青年组和中年组患者差异均有统计学意义（P〈0．05）；老年组患者在认知功能、社会功能、进食3个方面与其他3组患者差异均有统计学意义（P〈0．05）。结论不同诊断年龄对于食管癌患者生存质量的影响，主要表现在生理功能、认知功能、社会功能、进食以及食欲方面。OBJECTIVE The study focused on assessing the quality of life （QOL） of esophageal cancer patients of different ages at diagnosis to provide a theoretical basis for the implementation of personalized nursing for esophageal cancer patients. METHODS 951 patients with esophageal cancer residing in communities in north Henan were selected through the conve- nient sampling method and were surveyed and interviewed. The survey questionnaire included general demographic informa- tion, EORTC QLQ-C30 V3.0, and QLQ-OES18. The patients were divided into four groups based on their ages at diagnosis, including a youth group （ ~44 years old）, middle-age group （45-59 years old）, young-elderly group （60-74 years old）, and elderly group （ I〉 75 years old）. RESULTS Statistical difference in lost of appetite was observed between the youth group and the middle-age and young-elderly groups （P 〈 0.05）. Difference in physiological function between the young-elderly and elderly groups and the youth and middle-age groups was also statistically significant （P 〈 0.05）. Cognitive function, social function, and food intake of the elderly group were significantly different from those of the other 3 groups （P 〈 0.05）. CONCLUTION Age at diagnosis affects the quality of life of esophageal cancer patients, especially on their physical function, cognitive function, social function, food intake, and appetite.
