摘  要:1984年Porter等首次在控制性超排卵(COH)中使用促性腺激素释放激素激动剂(GnRH-a)抑制过早的内源性黄体生成素(LH)峰获得成功后,GnRH-a逐渐应用于垂体降调节。GnRH-a能有效降低内源性LH水平,使卵泡生长发育同步化,防止卵泡过早黄素化,从而提高卵母细胞质量。GnRH-a联合促性腺激素(Gn)的应用是COH中最常用的方案,尤以长方案最常用。Objective: To compare the clinical effects of six kinds of pituitary down regulation protocols with triptorelin in controlled ovarian hyperstimulation. Methods: A data of 162 cycles from Mar. 2011 to Sep. 2012 were retrospective analyzed. According to the total dose of short-acting triptorelin used,the experiment groups were divided into group A1 (0.3 mg), A2 (0.5 mg) and A3(0.7 rag). Triptorelin with 0.1 mg daily was given from the first or second day of the menstrual cycle for 3 days in group A1,5 days in group A2 and 7 days in group A3. A dose of 0.1 mg/day triptorelin was given from the mid-luteal phase or the late-luteal phase for 7 days in group A4. The control groups were divided into group B1 (long-protocol) and group B2 (short-protocol). The patients in group B1 were injected 1.25 mg depot triptorelin,and the patients in group B2 were given short-acting triptorelin 0. lmg/day from second day of the menstrual cycle to the day of HCG injection. The effects of pituitary down regulation were compared among the groups. Results: The LH suppression days were 11-15 days in group A1,16-20 days in group A2,A3 and A4, 27-31days in group Bl,and 11-15 days in group B2. The high-quality embryo formation rate in group A3 was higher than that in group B1 (P〈0.05). The high-quality embryo formation rate in group A1 was higher than that in group B2 (P〈0.05). Conclusions: The personalized micro down-regulation protocol with short-acting GnRH-a is an effective protocol to suppress LH level.
