胰腺非功能性神经内分泌肿瘤影像诊断

摘　　要：目的探讨胰腺非功能性神经内分泌肿瘤的影像表现。方法回顾性分析本院经手术及病理学检查确诊为非功能性胰腺神经内分泌肿瘤的6例患者的影像学资料,其中5例行CT检查,1例行CT及MRI检查,所有病例均行增强扫描,观察肿瘤的部位、大小、密度、边界、强化及转移特征。结果 6例患者共6个病灶,均位于胰体尾部,肿瘤均较大,最大径线5～10 cm,边界多较清楚,病变密度多不均匀,可见坏死囊变区,其中1例病灶以囊性成分为主,1例病灶可见斑片状钙化,增强扫描3例实性部分明显强化,2例实性部分轻至中度强化,1例分隔轻度强化,1例可以看到完整包膜结构,1例病变可见肝脏转移。结论胰腺非功能性神经内分泌肿瘤的影像学表现具有一定特点,可与胰腺其他恶性肿瘤相鉴别。Objective To study the imaging findings of nonfunctional pancreatic neuroendocrine carcinoma. Methods hnaging findings of 6 patients with nonfunctional pancreas neuroendocrine carcinoma proved histopathologically were retrospectively analysed. Five cases underwent CT examination,and 1 case underwent CT and MRI examination. All cases underwent contrast-enhanced scan, the location, size,density,contrast and transfer characteristic of the lesion were observed. Results Six patients had 6 lesions,all located in the pancreatic tail with the diameter line 5 - 10 em. Four cases had clear boundary, all cases had necrosis region, including lesions composed of cystic components in 1 case, calcification in 1 ,obvious enhancement in 3, moderate enhancement in 2, mild enhancement of internal space structure in 1, complete capsular structure in 1 ,liver metastasis in 3. Conclusion The imaging findings of nonfunctional pancreatic neuroendocrine tumors have certain characteristics, which can be differentiated from other malignant tumors in pancreas.
