原发及转移性结直肠腺癌中TTF-1的表达

摘　　要：目的探讨甲状腺转录因子-1(thyroid transcription factor-1,TTF-1)在结直肠腺癌中的表达及在诊断及鉴别诊断中的应用价值和局限性。方法结直肠腺癌160例,其中原发119例和发生肺转移41例,采用免疫组化EnVision两步法检测TTF-1的表达。结果 TTF-1阳性者15例,其中原发结直肠腺癌12例(10.1%)和肺转移腺癌3例(7.3%)。TTF-1的表达与患者年龄、性别、分化程度、有无黏液、发生部位及TNM分期均无相关性(P>0.05)。结论除甲状腺癌和肺腺癌外,TTF-1在结直肠腺癌中也存在一定比例的阳性表达,临床医师判断TTF-1阳性肿瘤的来源时,应考虑结直肠腺癌的可能。Purpose To investigate the expression of thyroid transcription factor-1 （TrF-1） in colorectal adenocarcinoma and the role of TTF-1 in pathological diagnosis and differential diagnosis. Methods Totally 160 cases of colorectal adenocarcinomas were involved in this study, including 119 primary colorectal adenocarcinomas and 41 lung metastases cases, and all of the cases were stained with TTF-1 antibody. Results Among 160 cases, 15 were positive, including 3 lung metastases （7.3%） and 12 primary colorectal adeno- carcinomas （10. 1% ）. The expression of TTF-1 was not relevant to age, gender, differentiation, mucus, colon sites and TNM stage （P 〉 0. 05）. Conclusion TTF-1 may be positive in a certain proportion of colorectal adenocarcinomas apart from thyroid carcinomas and lung adenocarcinomas, and therefore colorectum can not be ruled out when we define the origin of metastatic adenocarcinoma with positive expression of TTF-1.
