摘　　要：目的探讨甲状腺髓样癌(MTC)超声特征及其诊断价值。方法回顾性分析31个MTC结节大小、形态、边界、回声、内部钙化和淋巴结等参数,按形态分为规则型和不规则型,分别与滤泡状瘤(FTA)和乳头状癌(PTC)比较。结果规则型髓样癌(RMTC)和不规则型髓样癌(IMTC)比较,上下径(P=0.039),前后径(P=0.014),L/A比(P=0.041),边界(P=0.003),钙化(P=0.005),淀粉样物质沉积比例(P=0.021)差异有统计学意义。RMTC和FTA比较,钙化(P=0.001)差异有统计学意义,IMTC和PTC比较,上下径(P=0.022),前后径(P=0.003),L/A比(P=0.001),钙化(P=0.011),血流分布(P=0.000)差异有统计学意义。结论 RMTC超声征象与低回声FTA相似。IMTC除具备恶性肿瘤常见征象外,与PTC比较体积更大,呈卵圆形,钙化表现形式多变,血流更丰富。Objective Analyzed the uhrasonographic （US） characteristics in medullar thyroid carcinoma （MTC）. Methods Retrospectively reviewed the US characteristics in 31 MTCs, such as size, shape, border, echoge- nicity, calcification and vascularity were included in. According to shape, nodules were classified as regular MTC （RMTC） and irregular MTC （IMTC）. Results There were significant difference between RMTCs and IMTCs, in nodule longitudinal diameter （P = 0. 039）, anteroposterior diameter （P=0. 014）, L/A ratio （P = 0. 041）, border （P= 0. 003） and calcifications （P= 0. 005）. The calcifications was significantly difference between RMTCs and follicu- lar thyroid adenoma （FTA） （P=0. 001）. There were significant difference between IMTCs and PTCs, in nodule lon- gitudinal diameter （P=0. 022）, anteroposterior diameter （P = 0. 003）, L/A ratio （P = 0. 001 ）, calcifications （ P=0. 011） and vascularity （P=0. 000）. Conclusions The US characteristics in RMTC were similar with FTA. Apart from the US characteristics of suspicious malignance, IMTC tender to be larger, oval shape, with micro- or macro cal- cifications and hyper-vascularity.
