经皮下注射超声造影剂对不同分子分型乳腺癌腋窝淋巴结转移的诊断价值

摘　　要：目的探讨经皮下注射超声造影剂对乳腺癌不同分子分型腋窝淋巴结转移的诊断价值。方法选择女性乳腺癌患者95例,术前经肿块周围皮下注射超声造影剂探查前哨淋巴结并标记,与术后肿块免疫组织化学分子分型结果做对照分析。结果本研究95例患者中,超声造影发现乳腺癌分子分型中luminal B型腋窝淋巴结转率最高(70.8%),HER-2型(52.4.0%)次之,再者是三阴型(43.5%),luminal A型腋窝淋巴结转移率最低(29.6%),超声造影判断腋窝淋巴结转移的敏感度为81.3%,特异度为85.1%。结论经皮下注射超声造影剂对乳腺癌不同分子分型腋窝淋巴结转移有诊断价值。Objective To investigate the diagnostic value of ultrasound contrast agents injection through subcuta- neous in axillary lymph node metastasis of breast cancer patients. Methods Preoperatively, to probe and mark senti- nel lymph node through subcutaneous injection of ultrasound contrast agents, postoperatively take immunohistochemi- cal examination, to determine the molecular subtype. Contrast-enhanced ultrasound images of axillary lymph node of 95 women with breast cancer and their results of the molecular subtypes were compared analyszed. Results In 95 ca- ses, luminal B type breast carcinoma axillary lymph node metastasis rate is the highest （70.8%） with contrast-en- hanced ultrasound, HER2 type breast carcinoma take second place（52.4%）, then, triple negative breast carcinoma （43.5 %）, luminal A type breast carcinoma axillary lymph node metastasis rate is the lowest（29.6 % ）. The sensitivity and respectiely of axillary lymph node metastasis by contrast-enhanced ultrasound is 81.3%, 85.1% respectively. Conclusions Subcutaneous injection of ultrasound contrast agents has good diagnostic value in different molecular sub types of axillary lymph node metastasis in breast cancer patients.
