摘 要:目的:评估三种临床常用粘固方法对全冠边缘适合性的影响。方法:采用标准试验体制作标准代型18个,每一代型及熔模进行四点标记,进行包埋铸造,按标记点就位。将18个全冠随机分为A、B、C三组,采用玻璃离子粘接剂进行粘固,A组采用10kg直接静力加压粘固、B组采用直接动力加压粘固和C组采用分步动力加压粘固。每一个冠均采用10kg载荷加压至粘接剂完全硬固。每一个全冠在代型上粘固前后分别用奥林巴斯体视显微镜于四个标志点处对垂直边缘缝隙进行拍照,用Image-ProPlus 6.0进行测量后,计算粘固前后冠边缘缝隙的浮升量,使用SAS9.13对结果进行统计学分析。结果:粘固前后边缘浮升量:A组166±155μm,B组93±64μm,C组52±36μm,数据不符合正态性,秩和检验(χ2=12.58,P=0.0019)显示三组之间差异有统计学意义。结论:粘固会增加全冠边缘缝隙,分步动力加压粘固具有较佳的边缘适合性。Objective: To evaluate the marginal adaptation of full crowns using three different adhesion methods which are commonly used in clinic. Methods: Eighteen type 1V stone dies were pored with a silicone impression. Wax patterns for full crowns were fabricated by a traditional method. Four symbol sites were made in every pattern and die. All the patterns were invested and casting to fabricate crowns. The crowns were seated on the dies according their symbol sites. Then they were divided into A, B and C groups randomly. Glass ionomer cement was mixed and the crowns were cemented using the following methods in each group: A) 10 kg of static pressure cementation directly, B) dynamic pressure cementation direct- ly, C) dynamic pressure cementation in three steps. A 10 kg load was then maintained on the crowns until complete cement cure. Their vertical marginal gap was taken pictures at four points around each crown by Olympus optical stereornicroscope before and after cementation respectively, measured using Image-Pro Plus 6.0. All results were analyzed using SAS9.13. Results: The marginal discrepancy before and after cementation: A) 166±1551μm, B) 93± 64tμm, C) 52±36μm. Rank test (Х^2= 12.58, P= 0.0019) revealed significant differences among the groups. Conclusions: Cementation would increase the marginal gap. Dynamic pressure cementation in three steps was proved to provide the best marginal adaptation.
