摘　　要：目的分析鼻咽非霍奇金淋巴瘤的CT及MRI表现,以及颈部淋巴结受侵的特点,提高影像诊断及分期水平。方法回顾性分析经病理证实并有随诊记录的鼻咽非霍奇金淋巴瘤33例。结果 33例鼻咽部病变中,8例NK/T细胞型均表现为鼻咽壁轻度或轻微增厚,1例T细胞型表现为肿块样。14例弥漫大B细胞型,10例表现为肿块样,4例表现为弥漫增厚型;5例B细胞来源未分型,其中4例表现为肿块样,1例表现为鼻咽壁弥漫增厚伴鼻腔肿块。其余亚型及未分类共5例,3例表现为肿块样,2例为增厚型。后鼻孔受侵20例(60.6%);口咽壁受侵17例(51.5%);侵犯颅底骨6例(18.2%)。25例伴有颈部淋巴结受侵,多为双侧、多组受侵。鼻咽淋巴瘤与受侵淋巴结多表现为密度均匀,强化均匀、近似肌肉,边界清楚。结论不同病理类型鼻咽淋巴瘤影像表现各有一定特点,多数伴有颈部淋巴结受侵,以双侧、多组同时受侵多见。Objective To review the CT and MRI features of nasopharyngeal non-Hodgkin＇s lymphoma and character- istics of the involved cervical lymph nodes for improving imaging diagnosis and staging. Method A retrospective review was performed on 33 patients with pathologically proved nasopharyngeal non-Hodgkin＇s lymphoma. Result Of the 33 ca- ses with nasopharyngeal lesions, all 8 cases of extranodal NK / T-cell type showed mild or slight thickening of the nasopha- ryngeal wall and 1 case of T-cell type showed mass-like pattern. Among the 14 cases of diffuse large B-cell type, 10 cases appeared as mass-like pattern and 4 cases appeared as diffuse thickening. In the 5 cases of unclassified B-cell lymphoma, 4 cases were mass-like and 1 case appeared as diffuse thickening of nasopharyngeal wall accompanied with nasal mass. In the other subtypes and unclassified T/B case, 3 cases showed mass-like pattern, 2 cases appeared as diffuse thickening. In re- spect of the invasion, there were 20 cases in ehoanal （60. 6% ） , 17 cases in oropharyngeal wall （51.5%）, and 6 cases in skull base bone （ 18. 2% ）. 25 cases had cervical lymph node involvement, most of which were bilateral, multi-group involve- ment. The common CT and MRI features of nasopharynx and lymph nodes were of homogeneous density and enhancement sim- ilar to muscle with clear boundary. Conclusion Nasopharyngeal lymphomas of different pathological types show certain characteristics, most of which are accompanied with bilateral and multi-group cervical lymph nodes involvement.
