糖尿病消化性溃疡40例临床分析

摘　　要：目的探讨糖尿病患者消化性溃疡的特点。方法对40例糖尿病消化性溃疡患者(糖尿病组)与36例非糖尿病消化性溃疡(非糖尿病组)患者进行研究,分析两组的主要症状及胃镜诊断差别。结果糖尿病组患者腹部疼痛积分[(3.28±0.12)分]明显低于非糖尿病组[(5.26±0.35)分],差异有统计学意义(t=2.38,P<0.05);糖尿病组腹胀患者发生率(80.0%,32/40)明显高于非糖尿病组(25.0%,9/36),差异有统计学意义(χ2=8.318,P<0.05)。胃镜诊断:球部溃疡的发病率(15.0%,6/40)低于非糖尿病组(38.9%,14/36),而复合性溃疡发病率(37.5%,15/40)高于非糖尿病组(16.7%,6/36),差异均有统计学意义(χ2=4.876,P=0.027;χ2=4.669,P=0.031)。结论糖尿病消化性溃疡患者溃疡、腹胀症状较明显,疼痛症状相对较轻,复合性溃疡发病率较高,球部溃疡发生率相对偏低。Objective To study the characteristics of diabetic patients with peptic ulcer. Methods Research was con- ducted on 40 diabetic patients （diabetic group） with peptic ulcer and 36 non-diabetic patients with peptic ulcer（non-diabetic group） ,cardinal symptoms and difference of gastroscope diagnosis in the two groups were analyzed. Results The score of ab dominal pain in patients with diabetes mellitus （3.28＋0.12） was significantly lower than that in non-diabetic group （5.26±0.35）, and the difference between the two groups had statistical significance （t=2.38, P〈0.05 ）. The incidence of abdominal distention in diabetic group （80.0%, 32/40） was obviously higher than that in non-diabetic group （25.0%, 9/36）, and the difference between the two groups had statistical significance （A-2=8.318, P〈0.05 ）. Gastroscope diagnosis showed that the incidence of duodenal ulcer in diabetic group （15.0%, 6/40） was lower than that in non-diabetic group （38.9%, 14/36） with statistically significant difference （X^2= 4.876,P=0.027 ）, and the incidence of complex ulcer in diabetic group （37.5%, 15/40） was higher than that in non-diabetic group （ 16.7%, 6/36） with statistically significant difference （X^2=4.876, P=0.027 ;X^2=4.669, P=0.031 ）. Conclusion Abdominal distension in diabetic patients with peptic ulcer is more obvious with relative mild pain. The incidence of complex ulcer is higher and duode- nal ulcer is relatively lower.
