综合性医院精神科门诊初诊患者分析  

Analysis of new clients from psychiatric clinic in general hospital

摘　　要：目的研究综合性医院精神科门诊初诊患者的特征。方法对2006年1月1日至2012年12月31日初诊患者的性别、年龄、学历、婚姻、就诊目的、所患疾病及就诊方式等特征进行统计分析。结果共有9 551例患者纳入分析,男女比例1∶1.2;年龄3～93岁,19～40岁患者比例达到61.1%;学历主要分布在高中(29.4%)和大学(48.0%);已婚患者占大多数(58.5%);健康心理咨询占18.6%,精神障碍患者占81.4%,以神经症(40.1%)、心境障碍(34.5%)和精神分裂症(8.5%)为主;78.2%的患者主动就诊,被动就诊人数占3.5%,临床科室转诊18.3%,转诊科室以内科为主,主要来源于神经内科(18.1%)、心血管内科(17.6%)和消化内科(14.0%)。结论综合性医院精神科门诊初诊患者分布广泛,要加强普及精神疾病知识以及非精神专科临床医师对精神疾病的认识。Objective To study the characteristics of new clients from the psychiatric clinic in general hospital.Methods The clients′data were analyzed from the successively registered database of the psychiatric clinic from January 1,2006to December 31,2012.The data included sex,age,educational background,marriage,problems,diagnosis and the mode of how to see a doctor.Results A total of 9 551clients were included in the study,the sex ratio（male∶female）was 1∶1.2.The age ranged from 3to 93years old,the clients aging from 19to 40years old accounted 61.1%.48.0% of the clients had a university diploma and 29.4% clients had a high school diploma.The percentage of the married clients was 58.5%.The percentage of the health-counseling clients was 18.6%.81.4% of the clients suffered from mental disorder,with neurosis（40.1%）,mood disorder（34.5%）,and schizophrenia（8.5%）.78.2%of the clients with active treatment,3.5%of the clients with passive treatment,18.3%of the clients by clinical referral,most referral came from the department of medicine,with department of neurology（18.4%）,cardiovascular department（17.6%）,and digestive department（14.0%）.Conclusion The clients of psychiatric clinical in general hospital were widely distributed,the popularization of knowledge of mental disease and non psychiatric clinician awareness of mental illness should be strengthened.
