摘  要:目的探讨磁敏感加权成像(SWI)在脑微出血(CMBs)影像诊断中的价值。方法以SWI序列检测出CMBs的30例脑血管病患者为研究对象,通过常规MRI、T2*WI及SWI多序列对比研究,比较分析CMBs在各序列中的影像学表现、分布及检出敏感度。结果 SWI序列发现全部30例67个病灶,敏感度为100%。在SWI序列上表现为2.2~5.3mm大小不等的边界清晰、锐利的信号缺失灶。T2*WI序列共检出病灶18例51个,表现为信号缺失影,检出病灶的敏感度为76.1%。常规MRI的T1WI序列和T2WI序列共检出4例10个病灶(14.9%)。SWI序列与T2*WI序列、常规MRI序列对比差异有统计学意义(P<0.05)。结论 SWI序列对CMBs的检出敏感度高于常规序列及T2*WI序列,CMBs病灶的检出可以反映脑内微血管病变程度,对缺血性脑卒中的治疗及预后判断具有较为重要的临床应用价值。Objective To investigate the diagnosis value of susceptibility weighted imaging(SWI)in cerebral microbleeds(CMBs).Methods Thirty cases of cerebrovascular disease patients with CMBs detected by SWI sequence were adopted as the subjects.Multi-sequence comparative study by routine MRI,T2*WI and SWI was used to discuss the imaging features,distribution and sensitivity of CMBs.Results The SWI sequences detected 67CMBs in 30cases,the sensitivity was 100%.CMBs in SWI sequence showed 2-5mm round or oval clear and sharp boundary low signal loss,and the sequence of T2*WI found 51CMBs in 18cases,showing low signal loss,CMBs detection sensitivity of the T2*WI sequence was 76.1%;CMBs detections of the usual MRI such as T1WI,T2WI sequence were 10in 4cases(14.9%).SWI sequence,compared with T2*WI sequence and routine MRI sequence,showed significant difference(P 0.05).Conclusion The SWI sequence of CMBs detection sensitivity is higher than that of conventional sequence and T2*WI sequence.CMBs lesion detection can reflect brain microvascular lesions for great clinical application value.The SWI sequence has an important role to ischemic stroke treatment and prognosis.
