综合医院住院老年患者精神科会诊情况分析

摘　　要：目的:了解综合医院住院老年患者精神科会诊情况。方法:对本院老年患者的精神科会诊率、患者的科室分布、会诊原因、精神科诊断及随诊情况进行统计分析。结果:3年共622例次老年患者接受精神科会诊,年会诊率分别为1.06%、2.39%和2.60%,总随诊率为15.95%。特需病房的会诊率和随诊率最高(8.74%和22.95%),重症监护病房次之。会诊原因手术科室为"既往精神障碍",非手术科室为"目前情绪症状",重症监护病房为"目前精神症状",特需为"随诊"。精神科诊断为"神经症性、应激性障碍及焦虑状态"的比率最高(33.57%),与总随诊率比较,"精神分裂症及偏执状态"者随诊率(36.84%)明显增高,"神经症性、应激性障碍及焦虑状态"者(9.22%)明显降低(P均<0.05)。结论:综合医院老年住院患者有精神卫生服务及精神科连贯的针对性联络会诊服务的需求。Objective： To investigate the psychiatric consultation situation for senile inpatients in general hospital. Method： The psychiatric eonsuhation rate, distribution of patients sections, consultation cause, psy- chiatric diagnosis and follow-up situation were analyzed. Results：In the 3 years, there were 622 senile pa- tients receiving psychiatric consultation. Annual consultation rate was 1.06% ,2.39% and 2.60% respectively; the general follow-up rate was 15. 95%. The highest rate of eonsuhation and follow-up was special ward （g. 74% , 22.95% ） , the intensive care unit was second. The consultation cause was ＂previous mental disorder＂ in operation department, ＂now mood symptoms＂ in non-surgical departments,＂ mental symptoms＂ in intensive care unit and ＂follow-up＂ in special ward. The ratio of ＂neurosis, stress disorder and anxious state＂ （33.57%） in the psychiatric diagnosis was the highest. Compared with general foUow-up rate, the follow-up rate of ＂schizo- phrenia and paranoid state＂ （36.84%） was significantly higher and ＂neurosis, stress disorder and anxious state＂ （9.22%） was significantly lower （ all P 〈 0.05 ）. Conclusion： The senile inpatients in general hospital need mental health service and specialist mental coherent targeted contact consultation service.
