轻微早产儿视网膜病变患儿早期视力及屈光状态研究

摘　　要：目的研究轻微早产儿视网膜病变（retinopathyofprematurity，ROP）患儿早期视力及屈光状态，探讨ROP对早期视觉发育的影响。方法选取2009年3月～2011年3月我院眼科检查的ROP患儿共29例58只眼，同时选取正常早产儿46例共92只眼作为对照组，分别测量并对比两组视力及屈光状态。结果ROP组视力中位数为2．60，与对照组差异无统计学意义（P〉0．05）。ROP组等效球镜度中位数为3．00D，与对照组差异无统计学意义（P〉0．05）；散光中位数为0．75D，大于对照组，两者差异有统计学意义（P〈0．05）。早产儿视力与出生时体重及孕龄均成正相关（P〈0．05）。结论轻微ROP患儿在出生后早期主要呈远视状态，其视力发育及等效球镜度并未受到影响，但有更高散光风险。Objective To understand the visual acuity and refractive state in infants with mild retinopathy of prematu- rity （ROP） during their early life, investigate the affection of ROP for the visual development in early stage. Methods A total of 58 eyes of 29 infants with ROP who were examined in our hospital from March 2009 to March 2011 were included. While 92 eyes of 46 premature infants without ROP were included as control group. Visual acuity and re- fractive state were measured respectively. Results The median visual acuity of ROP group was 2.60, which was no sig- nificant difference with control group （P 〉 0.05）. The median spherical equivalent of ROP group was 3.00D, which was no difference with control group （P 〉 0.05）. The median astigmatism of ROP group was 0.75D, which was significant higher than control group （P 〈 0.05）. The visual acuity of premature infants was positive correlated with both birth weight and gestational age. Conclusion Most infants with mild ROP are hyperopia in early stage, their visual acuity and spherical equivalent are not affected, while they have more astigmatism risk.
