摘 要:目的:探讨广西肝癌高发区HLA-DRB1*08/16等位基因与广西原发性肝细胞肝癌(hepatocellular carcinoma,HCC)家族聚集性的关系.方法:在广西肝癌高发区配对选择肝癌高发家族成员、无癌家族成员各200例作为研究对象.应用聚合酶链反应/序列特异性引物(polymerase chain reaction/sequence specific p r i m e r,P C R-S S P)检测研究对象中H L ADRB1*08/16等位基因的频率,分析其与广西肝癌高发区肝癌家族聚集性的关系.结果:(1)HLA-DRB1*08等位基因在肝癌高发家族成员组携带率(6.0%)及无癌家族成员组的携带率(5.5%)经比较差别无统计学意义(2=0.046,P=0.830),而HLA-DRB1*16等位基因在肝癌高发家族成员组携带率(24%)明显高于无癌家族成员组(15.5%),该基因在两组中分布有显著差异(2=4.559,P=0.033);(2)随着高发家族中肝癌患者人数的增加,HLA-DRB1*08等位基因的表达频率无差别(5.4%vs 6.5%,2=0.120,P=0.729),而HLA-DRB1*16等位基因的表达频率明显升高(17.2%vs 29.9%,2=4.401,P=0.036).结论:HLA-DRB1*08可能与广西肝癌家族聚集无明显关系,HLA-DRB1*16可能与广西肝癌家族聚集有关.AIM: To investigate the association between HLA-DRB1*08/16 alleles and familial aggrega- tion of hepatocellular carcinoma (HCC) in a high-incidence area in Guangxi, China. METHODS: Two hundred subjects from families which have had two or more than two patients with hepatocellular carcinoma (FHHC group) and another 200 subjects from families which have not had patients with any malignant tumor (FNC group) were selected. Polymerase chain reaction/sequence specific primer was used to determine the frequencies of HLA-DRBl*08/16 alleles. RESULTS: There was no statistical difference in the frequency of HLA-DRBI*08 allele between the FHCC group and the FNC group (6.0% vs 5.5%, 2 = 0.046, P = 0.830). The frequency of HLA-DRBI*08 allele did not significantly increase with the increase in the number of patients with HCC in the FHCC group (5.4% vs 6.5%, 2 = 0.120, P = 0.729). However, the frequency of HLA-DRBl*16 allele in the FHHC group was significantly higher than that in the FNC group (24% vs 15.5%, X2 - 4.559, P = 0.033), and the frequency of HLA-DRB1*16 allele rose significantly with the increase in the number of patients with HCC in the FHCC group (17.2% vs 29.9%, 2 = 4.401, P = 0.036). CONCLUSION: There may be a significant cor- relation between HLA-DRB1*16 allele and fa- milial aggregation of hepatocellular carcinoma. However, HLA-DRBI*08 allele may not be as- sociated with familial aggregation of hepatocel- lular carcinoma.
