恶性肿瘤患者血型抗体分析及输血对策

摘　　要：目的研究恶性肿瘤患者体内抗体效价的变化及输血对策。方法选取该院2011年11月—2011年12月恶性肿瘤患者69例和同期健康体检者40例,采用抗体效价测定盐水法检测血清中的抗-A、抗-B IgM抗体效价。结果肿瘤组血清中的抗-A效价主要分布于4~256,抗-B效价主要分布于8~128;对照组抗-A效价主要分布于32~512,抗-B效价主要分布于16~512。肿瘤组抗-A、抗-B IgM抗体效价与积分低于对照组,差异有统计学意义（P〈0.05）。结论在给肿瘤患者交叉配血时,注意血型的正确鉴定,如遇正反定型不符又需紧急输血时,应给予O型洗涤红细胞,以确保输血安全。Objective To explore the changes of antibody titers of the patients with malignant tumor and correlative countermeasures for blood transfusion. Methods 69 patients with malignant tumor admitted in our hospital from November, 2011 to December, 2011 and 40 people conducted health examination in our hospital during the same period were selected as the tumor group and the control group, respectively. Anti-A and Anti-B IgM antibody titer of them were detected by antibody titer determination of brine method. Results Anti-A titers in serum of the tumor group were between 4 and 256,and anti-B titers between 8 and 128. Anti-A titers of the control group were between 32 and 512, and anti-B titers between 16 and 512. Anti-A and anti-B IgM antibody titer and integral of the tumor group were lower than those of the control group, the differences were statistically significant（P0.05）. Conclusion When patients with tumor are given cross matching, it should be pay attention to correct identification of blood type, such as in the case of grouping discrepancy, and the patients need for emergency blood transfusion, O washed red blood cells should be given to them, in order to ensure the safety of blood transfusion.
