原位微波消融与假体置换治疗四肢骨肿瘤近期临床效果比较

摘　　要：目的：比较原位微波消融与假体置换两种手术方法治疗四肢骨肿瘤的临床效果。方法：2008年1月至2012年2月，分别采用原位微波消融或肿瘤假体置换手术治疗四肢肿瘤40例，手术后1年，从肿瘤学、手术并发症、肢体功能三个方面进行对比。结果：平均随访26．5个月，微波消融组4例（4／30），假体置换组2例（2／10）死亡；肺转移2例，全部为假体置换组；复发5例，全部为微波消融组。微波消融组手术并发症发生率为15例，肿瘤假体置换组为2例。Enneking功能评分微波消融组12—28分，平均23分；假体置换组分17，29分，平均26分。结论：术后1年结果比较．两种治疗方法的总体生存率达到85％，均是可接受的保肢手术方法；微波消融组手术并发症高于假体置换组：假体置换组肢体功能优于微波消融组。Objective To compare the clinical effects of microwave ablation and prosthesis replacement for limb bone tumor. Methods From Jan 2008 to Feb 2012, 40 patients with limb bone tumor were treated by in situ microwave ablation or prosthesis replacement operation. 1 year after the operation, 3 aspects of the operation complications, such as oncology, complications, and limb function were evaluated and analyzed. Results During the mean follow-up of 26.5 months, 4 cases （4/30） in microwave group and 2 cases （2/10） in prosthesis replacement group were dead. Pulmonary metastasis was occurred in 2 cases in prosthesis replacement group ; and local recurrence was occurred in 5 cases in microwave group. 15 cases in microwave group and 2 cases in prosthesis replaeement group had complications. The Enneking functional score of microwave group was 12 - 28 points （average 23 points）, and that of prosthesis replacement group was 17 - 29 points （average 26 points）. Conclusion The Overall survival rates of in situ microwave ablation and prosthesis replacement for limb bone tumor reached 85%, which indicated that both of them were the acceptable method. The complication in microwave group was higher than that in prosthesis replacement group. The limb function of prosthesis replacement group was better than that of microwave group.
