妊娠期口腔颌面部多间隙感染的回顾分析

摘　　要：目的:总结分析妊娠期多间隙感染的临床特点和治疗方法 ,探索预防手段。方法:通过回顾分析2006年12月—2011年7月收治的15例妊娠期口腔颌面颈部多间隙感染患者的临床资料,对妊娠期口腔颌面部感染的病因、诊断和治疗等相关问题进行分析。结果:患者平均年龄28.6岁,孕周平均24.9周。对所有患者均采用口外切开排脓,每天换药,全身使用抗生素抗感染。患者平均换药天数为14 d,平均使用抗生素时间为8 d。治愈后分娩,其中1例早产,其余均足月产;7例人工分娩,8例自然分娩。新生儿各项指标均正常,新生儿指数平均为9.4,平均出生体重为3193 g。结论:妊娠期口腔颌面多间隙感染应早期诊断,早期对感染区切开引流。及时适当的处理,对妊娠结局和母婴健康无影响。PURPOSE： To analyze the clinical features and treatment of oral and maxillofacial and cervical infection in pregnancy and to explore preventive methods. METHODS： Fifteen patients from December 2006 to July 2011 were retrospectively reviewed. Aetiology, diagnosis, treatment and outcome were analyzed. RESULTS： The average age was 28.6 years, average gestational peroid was 24.9 weeks. Eleven cases were caused by maxillary third molar perieoronitis, 3 by maxillary molar apical periodontitis, 1 by parotitis. All patients underwent surgical drainage. One patient received tracbeotomy. Penicillin was applied to control infections. Average treatment duration was 14 days. All patients achieved good results. All neonates were healthy. Neonatal apgar score was 9.4 and average BM was 3193 g. CONCLUSIONS： Early surgical drainage remains the main treatment for oral and maxillofacial infection in pregnancy. Systemic application of antibiotics should be considered to control infection. This study did not find a definite association between oral and maxillofacial and cervical infection and adverse outcome of pregnancy.
