摘  要:目的应用二阶因子模型对不同健康状态的医务人员的工作应激进行评价。方法以参加2012年北京市宣武医院本院健康体检的在职医务人员(≥40岁)作为调查对象,并根据亚健康症状评价问卷和体检结果,将其分为三组(健康、亚健康和患病组)。采用一阶因子模型分析COPSOQ问卷与工作应激相关的5个维度间相关性,再用二阶因子模型建立COPSOQ综合评价指标,比较三组医务人员的工作应激状况。结果一阶因子模型中COPSOQ问卷5个维度间有较强的关联性,进一步建立的二阶因子模型拟合较好(RMSEA=0.056)。在二阶因子模型中,亚健康组工作应激消极因子得分最高(P<0.01),积极因子得分最低(P<0.01)。结论二阶因子工作应激消极因子和积极因子能很好地评价工作应激状况,并提示工作应激消极因子可能是亚健康的危险因素,工作应激积极因子可能是亚健康的保护因素。Objective To evaluate the work stress of medical staff in different health status by second-order factor analysis. Methods The survey was conducted in medical staff aged 40 years old and above who participated in the 2012 annual health medical examination from Xuan- wu Hospital. The subjects were divided into three groups ( health, subopti-mal health and illness groups) by the results of SHSQ-25 and health medi- cal examination, The first-order factor model to analyze the correlation a- mong the five dimensions of COPSOQ related to work stress. And the sec- ond-order factor model to establish COPSOQ evaluation, compared the work stress of medical staff stress in different health status. Results The results showed a strong correlation among the five dimensions of COPSOQ. And the second-order factor model fit well (RMSEA = 0. 056 ), The score of second-order factor showed the score of negative factor were highest ( P 〈 0.01 ) and the score of positive factor was lowest( P 〈 0.01 ) in suboptimal health group. Conclusion The second-order factor( negative and positive stress factor) were good to evaluation work stress. The results indicated the negative and positive factor may be the risk and protective factor of subopti- mal health respectively.
