脑膜瘤VEGF表达与瘤周脑水肿的相关性探讨

摘　　要：目的探讨脑膜瘤血管内皮生长因子(VEGF)表达与瘤周脑水肿的相关性。方法选择48例脑膜瘤标本,采用免疫组织化学法对其血管内皮生长因子(VEGF)表达进行检测,依照病理学、MR或CT表现判定肿瘤的良性/恶性程度、瘤周脑水肿分级并计分、肿瘤血管成分改变并计分。结果 48例脑膜瘤标本中,VEGF表达呈阳性者43例,阳性率为90%(43/48);组织学分级的Ⅰ级、Ⅱ级、Ⅲ级的VEGF阳性表达率分别为81%(26/32)、91%(10/11)、100%(5/5);VEGF表达水平和血管成分计分以及瘤周脑水肿计分均呈现出高度正相关性(P<0.005)。结论脑膜瘤VEGF表达水平能够明显反映脑膜肿瘤的良性/恶性程度,具有重要的指标意义;同时,脑膜瘤VEGF表达与瘤周脑水肿密切相关,脑膜瘤VEGF是导致瘤周脑水肿的重要因素之一。Objective It is to explore the correlation between the expression of VEGF and peritumoral brain edema in men- ingioma. Methods 48 cases of meningioma samples were chosen and their VEGF expression were detected by Immunohisto- chemical technique, benignancy and malignancy degree, classification and score of peritumoral brain edema, change and score of tumor vessels were evaluated according to pathology, MR and CT. Results In 48 cases of meningioma samples, there were 43 eases with positive expression of VEGF, and the positive rate was 90% （43/48） ; the positive rates of VEGF of histological grade I , II and III were 81% （26/32） , 91% （ 10/11 ） and 100% （5/5） ; the expression level of VEGF was highly correla- ted with Vascular components scores, peritumoral brain edema scores （P 〈 0. 005）. Conclusion The expression level of VEGF in meningioma can obviously show the benignancy and malignancy degree of the tumor, which has an important index significance; meanwhile, the expression is closely related with peritumoral brain edema, and VEGF is one of the important factors which lead to the edema.
