视可尼喉镜和纤维支气管镜在颈椎损伤患者经鼻气管插管中的应用比较

摘　　要：【目的】比较视可尼喉镜和纤维支气管镜在颈椎损伤患者经鼻气管插管的效果。【方法】拟在经鼻气管插管全麻下行择期手术的颈椎损伤患者60例，ASAI～Ⅱ级，年龄18～68岁，随机分为视可尼喉镜组（S组）和纤维支气管镜组（F组），每组30例。所有患者均采用颈托固定头颈于中立位，全麻诱导后，s组应用视可尼喉镜、F组应用纤维支气管镜引导经鼻气管插管。记录并比较两组插管成功率、插管时间、插管并发症、麻醉诱导及插管前后的血流动力学改变。【结果】两组插管成功率100％；两组一次插管成功率、插管时间、插管并发症及同时点血流动力学比较无显著性差异（P〉0．05）。【结论】颈椎损伤患者采用视可尼喉镜引导经鼻气管插管的应用效果与采用纤维支气管镜相当，且其携带方便、操作简便，是此类患者气管插管的较好选择。[Objective] To compare the efficacy of Shikani laryngoscope vs. fibreoptic bronchoscope used in nasotracheal intubation for patients with cervical spine trauma. [MethodslSixty ASA I - II patients（18- 68 years old） with cervical spine trauma scheduled for nasotracheal intubation under general anesthesia were randomly divided into Shikani laryngoscope group（group S, n =30） and fibreoptic bronchoscope group（group F, n =30）. Head and neck of all patients were fixed on neutral position with neck support. After anesthesia induction, group S underwent nasotracheal intubation with the guidance of Shikani laryngoscope, while group F underwent nasotracheal intubation with the guidance of fibreoptic bronchoscope. The success rate, time and complications of intubation and the changes of hemodynamics before and after anesthesia induction and intuba tion were recorded. [Results] The overall success rate of intubation in two groups was 100%. There was no significant difference in one-time success rate, the time and complications of intubation and hemodynamics at the same time point between both groups（ P 〉0.05）. [Conclusion] The efficacy of Shikani laryngoscope used in nasotracheal intubation for patients with cervical spine trauma is the same as fibreoptic bronchoscope. Shika ni laryngoscope is portable and easy to operate, so it is a good choice for nasotracheal intubation.
