河南高氟区氟牙症患者社会心理状况的评价

摘　　要：目的：评价高氟区不同程度氟牙症患者的社会心理状况,探讨氟牙症对人们社会心理的影响.方法：分层整群随机抽取河南省开封农村高氟区12岁和35 ～44岁当地居民共512名进行Dean氏氟牙症指数（DI）的调查,并从中筛选出不同程度氟牙症患者399例进行问卷调查,涉及5个心理分量表（对牙齿看法、主观幸福感、交往焦虑、自尊影响、行为改变）.结果：随着DI记分的增加,受检样本“对牙齿看法”和“主观幸福感”两个分量表得分呈上升趋势,但仅在“对牙齿看法”分量表中不同程度氟牙症患者间差异有统计学意义（P=0.019）.12岁组中不同程度患者间“对牙齿看法”差异有统计学意义（P =0.009）,且差异存在于“DI =0,0.5,1,2与DI =3,4”患者之间；成人组仅“交往焦虑”有统计学差异（P =0.020）.结论：高氟区氟牙症患者在“对牙齿看法”和“交往焦虑”方面的心理状况受到氟牙症程度的影响,存在年龄差异性.AIM：To study the effects of fluorosis on the psychosocial status of patients with fluorosis in the epidemic areas in Henan province.METHODS：Stratified,clustered and random sampling method was used for the study.512 subjects with dental fluorosis were classified by Dean＇ s index（DI）,399 of them with different types of fluorosis were involved in a psychosocial questionnaire investigation including attitude to teeth,subjective well-being,interpersonal or social anxiety,effect of self-esteem and impact on behavior.RESULTS：With the increase of DI score,the average value of subscales of ＂attitude to teeth＂ and ＂subjective well-being＂ was also increasing.Statistical difference of subscale average value was found only in subscale of ＂attitude to teeth ＂ （P =0.019）.Between mild group （DI =1,2） and severe group（DI =3,4） significant diference of ＂attitude to teeth＂ was found in 12-year-old subjects （P =0.009） and ＂interpersonal or social anxiety ＂ in 35-44-year-old subjects （P =0.020）.CONCLUSION：Attitude to teeth and interpersonal or social anxiety are the main psychosocial factors of fluorosis on patients in fluorosis epidemic areas,especially in severe group.
