摘　　要：目的:通过Meta分析方法评价粪便中肿瘤M2型丙酮酸激酶(tumor M2-pyruvate kinase,tumor M2-PK)的检测对结直肠癌的诊断价值。方法:计算机检索Pubmed、Cochrane library、Medline、中国知网期刊数据库、万方数据库和维普数据库等电子数据库,纳入以tumor M2-PK诊断筛查结直肠癌的临床诊断学实验,提取特征信息。采用Meta-Disc1.4软件进行汇总分析,绘制综合受试者工作曲线(SROC)。结果:15个研究的汇总灵敏度为77%(95%CI:74%～80%),特异度为77%(95%CI:75%～79%),阳性似然比为4.14(95%CI:3.19～5.38),阴性似然比为0.29(95%CI:0.25～0.34),诊断比值比DOR为16.31(95%CI:11.28～23.58)。受试工作者特征曲线下面积SROC AUG为0.858 2,Q=0.789 0。结论:应用ELISA法检测粪便tumor M2-PK对结直肠癌的诊断效力中等,该非侵入性检测方法可作为结直肠癌诊断的一种辅助方法。OBJECTIVE： To evaluate the diagnostic value of fecal tumor M2-pyruvate kinase on colorectal cancer. METHODS： Based on the principles and methods of Cochrane systematic reviews,literatures were searched in Cochrane Library, Medline, Pubmed, China National Knowledge Infrastructure （CNKI）, Wan fang databases, VIP databases. Articles related to the diagnostic value of fecal tumor M2-pyruvate kinase on colorectal cancer were included. Related journals were also searched manually. The software Meta-Dise I. 4 was used to review management and data analysis. RESULTS： The results of meta-analysis showed that the sensitivity of fecal tumor M2-pyruvate kinase test for diagnosis of colorectal canc er was 775（95%CI：74%- 80%）, specificity was 77% （95%CI： 75%- 79%）. The positive likelihood ratio was 4. 14 （95%CI：3.19-5. 38） ,negative likelihood ratio was 0. 29（95%CI：0.25-0.34）,and diagnostic odds ratio （DOR） was 16.31（95%CI：11.28-23.58）. The area under curve （AUC） of SROC curve was 0. 858 2 and Q index was 0. 789 0. CON- CLUSION：The performance of fecal tumor M2-pyruvate kinase detected by ELISA is moderate in the diagnosis of colorec tal cancer,it can be used as adjunct tool for non-invasive diagnosis of colorectal cancer.
