摘 要:研究鼠脑胶质瘤瘤周水肿及浸润组织的磁共振弥散加权成像(magneticresonancediffusionweightedimaging,MR-DWI)的特征,并与免疫组化水通道蛋白1(aquaporin-1,AQPl)对照,探讨瘤周水肿及浸润组织AQP1的表达与局部ADC值的相关性。方法 通过立体定向仪将C6肿瘤细胞接种至30只雌性Wistar大鼠的右侧尾状核。3~4周后,所有存活大鼠均行常规MRI、DWI、T1WI及T1-3D-bravo增强检查。借助FunctionTool软件对DWI数据后处理,选取3个等大ROI,获得相应部位的ADC、eADC值。选取与DWI相对应的瘤周水肿组织做AQP1免疫组化检查,Pearson 相关分析法检测局部ADC值和相应部位的AQP1阳性表达的累积光密度(IOD)值的相关性。结果 荷瘤大鼠中,瘤周水肿及浸润组织平均ADC 值为(0.86±0.03)×10-3 mm2/s,较对侧脑组织平均ADC值(0.80±0.04)×10-3 mm2/s升高,2组结果经两样本t 检验显示差别有统计学意义(t=3.01,P〈0.05)。而瘤周水肿及浸润组织平均eADC为0.42±0.03,较对侧脑组织eADC(0.45±0.03)降低,2组结果经两样本t检验显示差别亦有统计学意义(t=3.17,P〈0.05)。瘤周水肿及浸润组织AQP1表达的IOD值为79209.48±10766.45,Pearson相关分析显示,肿瘤水肿及浸润组织的ADC值与AQP1阳性表达IOD 值之间存在负相关(r=-0.61,P〈0.05)。结论 Wistar大鼠C6脑胶质瘤晚期阶段瘤周水肿及浸润组织的ADC值较正常脑组织升高,eADC值则较对侧脑组织减低,且ADC值与AQP1表达呈负相关。Objective To explore the magnetic resonance diffusion weighted imaging {DWI)features of peritumoral edema and infiltrated tissues in rat glioma,and the relationship between the aquaporin-1(AQP1)expression and regional ADC value. Methods C6 glioma cells were injected stereotactically into the right caudate nucleus of 30 Wistar rats. About 3-4 weeks after implantation, all survival rats were examined by conventional MRI, DWI, enhanced T1WI and T1-3D-Bravo (inversion recovery fast spoiled gradient echo, IR-FSPGR)seans. The DWI data was processed using the DWI Function Tool software to gain ADC and eADC images. Region of interest ( ROI)s were drawn avoiding hemorrhage, necrosis areas in 3 equal-sized (2 mm2 ) in peritumoral tissues and contralateral brain parenchyma to gain ADC value and eADC value. Peritumoral tissue corresponding to the level of the tumor in ADC images were examined using immunohistochemical staining for AQP1. The correlation between regional ADC value and the IOD value of AQP1 expression was analyzed by Pearson test. Results Mean ADC value of peritumoral edema and infiltrated tissues was (0.86±0.03)×10^-3 mm2/s,which was significantly higher than the value of the contralateral brain tissue[ ( 0. 80 ± 0. 04 ) ×10-3 mm2/s]. Two independent-sample t test of two groups showed statistically significant difference(t=3.01,P〈0.05). While the mean eADC value was (0.42±0.03),which was lower than mean eADC value (0.45±0.03)of the contralateral brain tissue.
