内镜清创联合药物注射治疗消化性溃疡对患者血清IL-17和IL-25水平的影响研究

摘　　要：目的探讨内镜治疗对消化性溃疡患者血清IL-17及IL-25的影响。方法选取本院收治的消化性溃疡患者84例,随机分为观察组和对照组各42例;对照组采用常规药物疗法,观察组行内镜清创加局部药物注射,并分别于治疗前、治疗后1 d、7 d、14 d取患者外周血,以ELISA试剂盒测定IL-17及IL-25的水平。结果经2个疗程的治疗,观察组治愈41例(97.6%),对照组治愈31例(73.8%),观察组的临床治愈率显著高于对照组(P<0.01)。治疗前2组患者的IL-17及IL-25水平均无显著差异(P>0.05),治疗后1 d,观察组的IL-17及IL-25水平显著高于对照组(P<0.05);治疗后7 d,观察组的IL-17及IL-25水平显著低于对照组(P<0.05);治疗后14 d,观察组的IL-17及IL-25水平显著低于对照组(P<0.01)。对照组治疗前后各时间段的IL-17及IL-25水平呈依次降低趋势;观察组治疗前后各时间段的IL-17及IL-25水平表现出瞬时升高后持续降低的趋势。结论内镜治疗消化性溃疡,在彻底清除肉芽组织的同时,提高了药物治疗的靶向性,可有效抵抗病原菌感染,抑制溃疡面促炎介质的分泌,具有较高临床应用价值。Peptic ulcer is a common disease in stomach and duodenum with acute bleeding or perforation as first symptoms which could cause long-term chronic inflammatory reaction. We aim to explore the impact of endoscope treatment on the serum levels of IL-17 and IL-25 of patients with peptic ulcer through this study in which 84 cases of patients with peptic ulcer were selected and randomly divided into two 42 cases of observation group and 42 cases of control group. Patients in control group were treated with conventional drugs, while those in observation group were treated with endoscopic debridement and drug injection; peripheral blood of patients were extracted respectively before treatment, as well as 1 d, 7 d, and 14 d after treatment to determinate IL-17 and IL - 25 levels with ELISA kit. We found that cure rate of observation group was significantly higher than that of control group （P〈 0.05）. Further more, there was no significant difference between groups of patients in IL-17 or IL-25 levels （P 〉 0.05） before treatment; 1 d after treatment, both the IL-17 level and the IL-25 level of patients in observation group were significantly higher than that in control group （P〈 0.05）. Seven days after treatment, both the IL-17 level and the IL-25 level of patients in observation group were significantly lower than that in control group （P 〈 0.05）, and 14 days after treatment, the difference enlarged （P 〈 0.01）. Our research result demonstrates that endoscopic treatment has high clinical application value for peptic ulcer with remarkable advantages of removing granulation tissues and rising targeting effects of drug therapy, which is effective to resist against pathogens infection and suppress the secretion of proinflammatory medium.
