摘 要:目的对比分析PET—CT多显像剂检查与MRI在胶质瘤诊断的临床应用价值。方法回顾分析初诊疑为胶质瘤患者52例,以病理学结果为金标准分析PET—CT检查及MRI检查在胶质瘤诊断的临床应用价值。结果32例患者经病理证实为胶质瘤;PET—CT检查出现2例假阳性,2例假阴性;MRI检查出现4例假阳性,13例假阴性;两者之间有统计学意义(McNemar检验,P=0.007〈0.05)。PET-CT多显像剂显像在胶质瘤诊断灵敏度为94%(30/32),特异性为90%(18/20),准确率为92%(48/52);MRI检查诊断的灵敏度为59%(19/32),特异性为80%(16/20),准确率为67%(35/52)。结论PET.CT多显像剂显像在胶质瘤诊断的灵敏度、特异性及准确率均显著高于MRI检查,在颅内胶质瘤诊断中有一定的优势。Objective To compare the diagnostic value of PET-CT and MRI in diagnosing of gliomas. Methods To compare the sensitivity and specificity of PET-CT with lSF-FDG and 13N-NH3/nC- MET with MRI, the results of MRI and PET-CT imaging of 52 patients with suspicious gliomas were retrospectively reviewed. All cases were confirmed by operation and histopathology. Results Among the 52 cases, 32 patients were verified with glioma. In PET-CT imaging there were only 2 false positive cases and 2 false negative cases. In contrast MRI produced 4 false positive cases and 13 false negative cases. The occurrence ratio is significantly different between these two entities ( McNemar test, X2 = 104, P = 0. 007 〈 0. 05 ). PET-CT displayed sensitivity 94% (30/32), specificity 90% ( 18/20), and accuracy 92% (48/52) in the diagnosis of gliomas. But MRI had the sensitivity 59% ( 19/32 ), specificity 80% ( 16/20), and accuracy 67% (35/52). Conclusions Compared with MRI, PET-CT with ISF-FDG and 13N-NH3/11 C-MET were demonstrated with higher sensitivity, specificity and accuracy than MRI, and would have more advantages in the diagnoses of intracranial gliomas.
