摘 要:目的:初步探讨全口义齿上颌后牙区堤颊舌向位置对下颌Piezography中性区印模颊舌向位置的影响。方法:在临床门诊选择无牙颌患者5例,设定2种全口义齿上颌后牙区堤颊侧缘位置:1)与上颌基托颊侧缘在咬合平面投影线重合(B-0);2)位于上颌基托颊侧缘在咬合平面投影线偏舌侧3mm(B-3)。采用Piezography发音法制取下颌中性区印模,通过数字图像处理,测量印模面磨牙区(M),磨牙与双尖牙交界区(MP),双尖牙区(P)及前牙正中部(AC)的唇颊舌向位置变化,并使用配对T检验进行统计学分析(显著性水平α=0.05)。结果:在B-3条件下,MP和P的颊侧缘及颊舌中点的位置较B-0条件下的更偏向舌侧(P<0.05),且更接近牙槽嵴顶,M和AC各部位的位置虽与B-0条件下的无显著差异,但存在偏向舌侧的倾向。结论:全口义齿上颌后牙区堤偏向舌侧设定可引起下颌Piezography中性区印模颊侧缘同向偏移,有利于下颌人工牙排列更接近于牙槽嵴顶。Objective: To investigate the relationship between buccolingual position of maxillary wax rim and mandibular neutral zone position while using piezography technique to make impressions for complete denture patients. Methods: Five edentulous patients were enrolled in this experiment. Two kinds of buccal edges of maxillary posterior wax rim were set for each patient. One buccal edge is the projection of the maxillary denture base on occlusal plane (B-0), and another was located 3 mm lingual of the projection line (B-3). After getting mandibular neutral zone impressions by Piezography technique, the bucco-lingual postion of molar region (M), junction region be- tween molars and premolars (MP), premolars region (P) and anterior region (AC) were analyzed by digital image measurements. Statistical analysis was performed by using the paired t test (alpha=0.05). Results.. About image analysis results from mandibular impressions of the group B-3, the buccal edge and buccolingual midpoint of MP and P were more deflected lingually than those of B-0, which was closer to alveolar ridge crest than those of group B-0, and the results had statistic differences (P^0.05). Although there was no statistic differences for the M and AC region, the tendency of data was showed that the buccal edge of impression were deflected toward lingual side in the image analysis results of group B-3. Conclusion: Deflecting the buccal edge of the maxillary wax rim lingually while taking complete denture impressions, one may get correspondingly lingual deflection of mandibular buccal impression edge using Piezography neutral zone technique and therefore may make the artificial teeth arrangement more appropriate, which is close to residual alveolar crest.
