根尖定位仪定位根管纵折的体外研究

摘　　要：目的:研究两种不同多频根尖定位仪定位根管纵折的能力及相关影响因素。方法:在釉牙骨质界处截除43颗单根管下颌前磨牙牙冠。在牙根冠方制备水平参考平面,牙根的近中或远中面打磨出平行于牙根长轴的水平面。随机用3种不同规格的金刚砂片在水平面上沿根管长轴制备一条联通根管的水平槽。根尖定位仪PROPEXⅡ与JustyⅡ在固定测量装置上分别定位水平槽上端顶点并计算测量锉尖与顶点的距离(DEUL)。在显微镜下测量水平槽的大小以及上下顶点距离根尖和冠方参考平面的距离。结果采用Mann-Whitney U检验和相关分析。结果:PROPEXⅡ和JustyⅡ测量DEUL值间无显著性差异,平均值分别为0.904mm和0.711mm,分别定位60.46%(n=26)和65.21%(n=28)测量锉尖距离槽上顶点冠方1.0mm范围内。两种根尖定位仪测量值与水平槽的面积和宽度显著性相关(P<0.05)。结论:在本研究条件下,两种多频型根尖定位仪不能精确定位不完全纵折,但可以为临床上牙根不完全纵折早期诊断提供参考。根尖定位仪测量值变化与纵折大小相关,与其位置无关。Objective： To evaluate two different multi-frequencies based apex locators in detecting vertical fracture and related influencing factors. Methods： Forty-three extracted mandibular premolar were decoronated at cemento-enamel junction. The mesial or distal surface of the root was prepared to be a flat plane. Then an incomplete vertical fracture （IVF） was simulated by creating a straight incision on the flat plane along the root canal long axis by three carborundum discs with different size, and the distance between file tips was determined by two EALs mounted in a measuring unit and the upper lever of the incision （DEUL） was calculated. The position and size of the, IVF were measured under the stereomicroscope. The results were subjected to Mann--Whitney U test and correla- tion analysis. Results： No significant difference was found between the PROPEX II （0. 904±mm）and Justy II （0. 711±mm）. The file tips was located 60.46% （n=26） by the PROPEX 1] , and 65.21% （n=28） by the Justy II within 1.0 mm short of the simulated IVF respectively. The area and width of the incision were significantly correlated to the measurements of EALs （P〈0.05）. Conclusion： In the situation of this study, these two apex locators were unreliable to detect the IVF, but potential to provide early diagnosis of IVF. The measurements of apex locators were significantly influenced by the size, but not the position of the IVF.
