第一双尖牙根管治疗术后集中片切矫治中度拥挤牙[牙合]畸形“临界病例”的临床应用研究

摘　　要：目的：探讨第一双尖牙根管治疗后集中片切矫治中度拥挤牙牙合畸形＂临界病例＂的矫治效果。方法：对牙拥挤度3.0~6.0mm的＂临界病例＂用第一双尖牙根管治疗后集中片切,常规直丝弓矫治法完成治疗（集中片切组）。20例＂临界病例＂选择拔牙矫治（拔牙矫治组）和16例＂临界病例＂选择邻面去釉矫治（邻面去釉组）。观察牙列排齐整平时间、间隙关闭时间、完成矫治时间;矫治期间和矫治结束后牙齿过敏、龋齿发生率。结果：牙列排齐整平时间无显著差异。关闭间隙时间和完成矫治时间上集中片切组与拔牙矫治组有显著差异（P〈0.05）。集中片切组与邻面去釉组牙过敏及龋齿发生率有显著差异（P〈0.05）。结论：第一双尖牙根管治疗后集中片切可为中度拥挤牙牙合畸形＂临界病例＂矫治提供足够间隙,缩短拔牙矫治关闭间隙时间,减少牙过敏、龋齿发生的风险。Objective： To study clinical application of borderline cases suffering from crowded and misaligned teeth corrected with the help of concentrate slicing after the root therapy of the first bicuspid. Methods： 24 borderline cases which have a dental congestion varying from3.0mm to 6.0ram are concentrate slicing after the root therapy in order to have the desired internal gap. They are treated with the routine straight wire appliance （the concentrate slicing group）. At the same time, 20 borderline cases having misaligned teeth received extraction treatment （the ex- traction group） and another 16 cases are treated by means of interproximal enamel reduction （the interproximal en amel reduction group）. A contrastive study has been carried out among the three different groups with regard to the time of supplanting and leveling the dentition, the time of space closure, the duration time of treatment as well as the incidence of dentin sensitivity and tooth decay complication during and after the treatment. Results： When it comes to the average time needed for alignment and leveling, there is no statistical significance between the concentrate slicing group and the control groups. However, when it comes to the time needed for complete closure and treatment, there does exist significant difference between the concentrate slicing group and the extraction group since （P〈0.05）. As far as dentin sensitivity is concerned, there is significant difference when compared with the interproximal enamel reduction group since. As for the incidence of tooth decay complication, there is also significant difference when compared with the interproximal enamel reduction group since （P〈0.05）. Conclusion： The concentrate slicing after the root therapy of the first bicuspid proves to be quite effective for the borderline cases suffering from crowded and misaligned teeth.
