核素联合美蓝定位法与钢丝定位法在切除乳腺病灶中的对照研究

摘　　要：目的:探讨核素联合美蓝定位法较传统钢丝定位法在治疗不可触及的乳腺病灶中的优势。方法:选取钼靶片显示微小钙化灶但触诊为阴性的患者100例,随机分为核素与美蓝联合定位组实验组50例,钢丝定位组对照研究组50例。分别对两组患者的病灶切缘状态,标本重量,手术切口长度,手术时间,出血量,术后并发症及患者满意度等进行对比研究。结果:两组患者的乳腺肿块均完整切除。实验组的病灶切缘状态,标本重量,手术切口长度,手术时间和患者心理满意度均明显优于对照组(P<0.05);两组的出血量及并发症的发生率比较差异无统计学意义(P>0.05)。结论:核素联合美蓝定位法较传统钢丝定位法具有明显的优势,值得临床推广应用。Objective To investigate the superiority of the combination with methylene dye localization on nonpalpable breast lesions contrasting traditional wire localization. Methods A total of 100 cases with mammography shows tiny calcifications but palpation were treated with the methylene dye Localization on nonpalpable breast lesions contrasting traditional wire localization.All cases were divided into two groups randomly,namely experimental group patients with methylene dye Localization （n=50）,and the control group with contrasting traditional wire Iocalization（n=50）.Percentage of close or involved surgical margins,mean weight of specimen （gram）,mean length of incision （mm）,duration of operation,blood loss,degree of satisfaction and postoperative complications in this two groups were compared. Results Better results were observed in treatment group than control group in terms of the following indicators：surgical margins,mean weight of specimen （gram）, mean length of incision （mm）,duration of operation,degree of satisfaction.The differences were significant （P〈0.05）.There was no statistical difference between two groups in terms of post-operative complications and blood loss （P〉O.05）. Conclusions Combination with methylene dye localization was advantageous with shorter duration of operation process, low negative rate of surgical margins,small weight of specimen and higher satisfaction rate over traditional wire localization on nonpalpable breast lesions, that is worthy of clinical application.
