淋巴管造影评价兔乳腺前哨淋巴结的实验研究

摘　　要：目的:探讨淋巴管造影评价肿瘤转移性及炎性前哨淋巴结(SLN)的可行性。方法:采用VX2肿瘤组织块悬液和新鲜蛋黄乳胶皮下注射法制作VX2乳腺癌和炎性兔模型。CT/MR淋巴管造影(CT/MR-LG)图像上SLN是指自对比剂注射部位至腋窝方向上最先引流的淋巴结,其余淋巴结为n-SLN(n-SLN),与SLN活检(SLNB)对照。结果:①CT-LG中,荷瘤组SLN及n-SLN较炎症组的大,但两者无统计学差异;淋巴结边缘出现充盈缺损在转移性SLN(2/2)多见。②MR-LG中,荷瘤组和炎症组SLN的大小及形状均无统计学差异;转移性SLN的长径大干非转移性的,且边缘环状高信号多出现在非转移性SLN中(2/10 vs 13/18),两者均有统计学意义。③对照SLNB结果,荷瘤组和炎症组SLN均较SLNB获得的SLN小,但MR-LG中SLN的大小较SLNB获得的差别大,且在荷瘤组中两者均有统计学差异。结论:转移性及非转移性SLN在CT-LG及MR-LG图像上有一定的特点。Purpose： To evaluate the feasibility of sentinel lymph node （SLN） mapping by using the method of lymphography. Methods： VX2 rabbit breast cancer model and rabbit inflammation model were established by injection of tumor tissue mass suspension or the yolk colloid. On MR/CT-LG images, the first lymph node（s） in the lymphatic drainage pathway was/were defined as the SLN（s）, and the others were n-SLN. The results of MR/CT-LG were compared with the results of SLN Biopsy. Results： On CT-LG images, the size of SLN and n-SLN in bearing tumor rabbits were bigger than that of inflammation group, and there was with no statistical significant difference; irregular filling defect at the margin was more frequent in metastatic SLN （2/2）. On MR-LG images, the size of SLN, as well as the shape were with no statistical significant difference between the tumor and inflammation group; when compared these results with the SLN biopsy pathology result, the long diameter of metastatic SLN was bigger than that of non-metastatic SLN, and ring-like enhancement was more frequently found in non-metastatic SLN （2/10 vs. 13/18）, which were with statistical significant differences. When compared with the results of SLN biopsy, the size of SLN in bearing tumor and inflammation rabbits were smaller than that of SLN achieved by SLN biopsy, and the difference was bigger in MR-LG than that of in CT-LG in tumor bearing group rabbits. Conclusion： Metastatic and inflammation SLN has some characteristics on CT-LG and MR-LG images.
