微波消融联合经脐静脉介入化疗治疗结肠癌肝转移瘤临床研究

摘　　要：目的探讨经皮微波消融(microwave ablation,MWA)联合经化疗泵脐静脉介入化疗治疗结肠癌肝转移瘤的临床价值。方法对83例结肠癌肝转移瘤行腔镜下结肠癌姑息性切除术后患者,按随机数字表法分为实验组(43例)和对照组(40例),两组均在结肠癌姑息性切除的同时完成了经脐静脉插管皮下化疗泵埋置术,实验组行MWA+经化疗泵脐静脉介入化疗,对照组单纯经化疗泵脐静脉介入化疗,治疗后进行随访,评价疗效和生存率。结果治疗后实验组CEA转阴率为76.4%,显著高于对照组的45.1%(P<0.05)。实验组1年、2年生存率分别为88.3%、60.5%,对照组分别为67.5%、32.5%,实验组优于对照组(P均<0.05),实验组3年生存率也高于对照组,但差异无统计学意义(P>0.05)。两组均无严重并发症。结论 MWA联合经脐静脉介入化疗在结肠癌肝转移瘤的治疗中占有重要的位置,是一种安全有效的治疗方法。Objective To investigate the significance of microwave abtation（MWA） combining umbilical vein interventional chemotherapy（UVIC） treating hepatic metastasis post laparoscopie palliative resection of colon cancer. Methods A total number of 83 cases with hepatic metastasis post laparoscopJe palliative resection of colon cancer were collected and they were randomized into experimental group（ n =43） and control group（ n =40 ） with random digits table.Both groups were inserted with UVIC pumps while undertaking laparoscopic palliative resection of colon cancer.The experimental group received UVIC ＋ MWA and control group received single UVIC.The patients were followed up and the efficacy and survival rates were analyzed.Results The rate of carcino-embryonic antigen（CEA） becoming negative in the experimental group was higher than that of the control group with 76.4% against 45.1 to, （ P 〈0.05）. The survival rates were 88.3% ,60.5% and 38.2% in 1 year,2 year and 3 year, in the experimental against 67.5%,32.5% and 0.5%/o in the control group（ P 〈0.05）.There was no severe complication in both groups. Conclusion MWA combining UVIC plays important role in treating hepatic metastasis post laparoscopic palliative resection of colon cancer ,which is safe and effective.
