血清C-反应蛋白和癌胚抗原水平在肺癌诊断中的价值

摘　　要：目的探讨C-反应蛋白(CRP)和癌胚抗原(CEA)对肺癌的诊断价值。方法选取2011年6月-2012年12月于温州医科大学附属第二医院就诊的65例肺癌患者作为肺癌组,同期收治的60例良性肺部疾病患者作为良性肺病组,同期行常规体检的健康人50例作为健康对照组。3组研究对象抽取静脉血,对血清中的CRP和CEA含量进行检测和分析。结果血清CRP和CEA水平肺癌组显著高于良性肺病组,良性肺病组显著高于健康对照组,差异具有统计学意义(P均<0.05);肺癌患者中血清CRP和CEA表达水平Ⅳ期显著高于Ⅱ期+Ⅲ期,Ⅱ期+Ⅲ期又显著高于Ⅰ期,差异均具有统计学意义(P<0.05);CRP与CEA联合检测诊断肺癌的敏感性为67.7%,特异性为95.4%,准确性为81.5%,均优于单个指标对肺癌的诊断。结论血清CRP和CEA水平对于肺癌的诊断有重要的价值,两者联合检测肺癌具有敏感性高和特异性好等优点,显著提高了肺癌诊断的准确率,值得临床推广。Objective To explore the diagnostic value of C-reactive protein （CRP） and carcinoembryonic antigen （CEA） in the diagnosis of lung cancer. Methods 65 patients with lung cancer received in our hospital were selected as lung cancer group, 60 patients with benign lung disease were selected as benign lung disease group, and 50 cases of healthy people were selected as healthy control group. Patients in three groups were treated with exsanguinations, and serum CRP and CEA levels were detected and analyzed. Results Serum CRP and CEA levels in the lung cancer group were significantly higher than those in benign lung disease group, and the serum CRP and CEA levels in benign lung disease group were significantly higher than those in the healthy control group, the difference was statistically significant （P 〈 0. 05） ;The expression levels of serum CRP and CEA in lung cancer patients were gradually increased with the clinical stage （ stage Ⅳ was significantly higher than stage Ⅱ ＋ Ⅲ, stage Ⅱ ＋ Ⅲ was significantly higher than stage 1 ）, the differences both were statistically significant（ P 〈 0.05 ） ;the lung cancer sensitivity of CRP combined CEA for the diagnosis of lung cancer was 67.7% ,specificity was 95.4% and accuracy was 81.5% ,which better than the diagnosis with single indicator. Conclusion Serum CRP and CEA levels had significant value for the detection of lung cancer and worthy of clinical.
