超声双重造影在判定食管胃底静脉曲张中的应用

摘　　要：目的探讨口服造影剂和静脉注射超声双重造影(DCUS)在食管胃底静脉曲张诊断中的应用价值。方法食管胃底静脉曲张患者28例为观察组,健康志愿者22例为正常对照组。采用DCUS观察两组食管下段双层黏膜和黏膜下层厚度(以下简称黏膜厚度)、胃壁静脉及胃周围静脉曲张情况。结果①DCUS诊断食管胃底静脉曲张的敏感性和特异性分别为89.7%和95.2%;②观察组中轻度曲张者黏膜厚度为(5.82±0.56)mm,中重度曲张者黏膜厚度为(8.31±0.42)mm,均大于对照组黏膜厚度(2.80±0.70)mm,差异有统计学意义(P﹤0.01)。结论 DCUS诊断食管胃底静脉曲张的敏感性和特异性高,操作简便易行,有望成为临床筛查食管胃底静脉曲张的一种新方法。Objective To explore the application value of combining oral ultrasonic contrast agent with SonoVue to enhance uhrasonography for the diagnosis of esophageal gastric varices. Methods Twenty-eight patients with esophageal gastric varices were enrolled as observation group, twenty-two healthy volunteers were taken as control group. The thickness of the lower esophageal bilayer mueosa and submucosa, gastric wall veins and surrounding veins were observed by double contrast-enhanced ultrasound（DCUS）. Restdts （~）The sensitivity and specificity of DCUS for the diagnosis of esophageal gastric varices were 89.7% and 95.2%, respectively. （~）In the observation group, the thickness of the lower esophageal bilayer mueosa in patients with mild- moderate esophageal gastric variees was （5.82_＋0.56）mm, and （8.31_＋0.42）mm in patients with severe esophageal gastric variees, which were both bigger than that in the control group （2.80 _＋ 0.70）mm （P 〈 0.01 ）.Conclusion DCUS is highly sensitive and specific for diagnosis of esophageal gastric varices, and it might be a new screening tool.
