胃癌患者外周血中单个核细胞及CD3^+CD8^(low)T细胞中ILT3的表达及意义

摘　　要：目的探讨免疫球蛋白样转录子3(ILT3)在胃癌患者外周血单个核细胞(PBMC)和CD3+CD8lowT淋巴细胞亚群中的表达,并分析其表达与胃癌生物学行为和临床病理学特征的关系。方法选择80例胃癌患者及40例健康人对照者为研究对象,用实时荧光定量PCR检测PBMC的ILT3 mRNA的表达,流式细胞术(FACS)检测外周血CD3+CD8lowT淋巴细胞亚群中ILT3的表达,并结合临床病理特征进行分析。结果胃癌患者与健康人对照者PBMC的ILT3 mRNA相对表达量分别为8.465±0.674和6.234±0.556,差异有统计学意义(t=2.160,P<0.05);低分化和中、高分化胃癌患者PBMC的ILT3 mRNA相对表达量分别为8.217±0.433(n=66)和3.656±0.195(n=14),差异有统计学意义(t=4.802,P<0.01);肿瘤未侵及深肌层与侵及深肌层比较,PBMC的ILT3 mRNA相对表达量分别为3.495±0.243(n=24)和7.316±0.404(n=56),差异有统计学意义(t=5.971,P<0.01)。胃癌患者和健康人对照者CD3+CD8lowT淋巴细胞亚群ILT3阳性率为(75.53±2.20)%和(12.83±0.94)%,差异有统计学意义(t=19.67,P<0.01)。结论胃癌患者PBMC的ILT3 mRNA表达与肿瘤细胞分化和浸润程度有关。ILT3在胃癌患者CD3+CD8lowT细胞亚群的表达增强,提示其可能参与肿瘤的免疫逃逸。Objective To investigate the expression of immunoglobulin-like transcript 3 （ ILT3 ） in peripheral blood mononuclear cells （PBMC） and CD3^＋CD8^lowT cell subset from gastric cancer patients and analyze the relationship between biological behavior of tumor cells and clinicopathoiogical features. Methods The expressions of ILT3 mRNA in PBMC of 80 patients with gastric cancer were de-termined by real-time fluorescent quantitative PCR and ILT3-positive CD3^＋CD8^lowT cells were measured by flow cytometry （FACS）. The correlations of ILT3 expression with the clinicopathoiogical features were analyzed. Results The expression of ILT3 mRNA in PB-MC of gastric cancer patients was remarkable higher than that of healthy controls, and the relative expressions were （8. 465 ± 0.674 ） and （6.234 ± 0.556） respectively. The expression of ILT3 mRNA in the patients with poorly differentiated gastric cancer was signifi-candy higher （8. 217 ± 0. 433 ） than that in the patients with well-differentiated patients （ 3. 656 ± 0.195 ） （ t = 4. 802, P 〈 0.01 ）. The expression of ILT3 mRNA in the patients with tumor invaded in deep muscularis was significantly higher （7.316 ± 0. 404, n = 56 ） than that in the patients with non-invaded deep museularis （ 3. 495 ± 0. 243, n = 24） （ t = 5. 971, P 〈 0.01 ）. ILT3-positive CD3^＋CD8^lowT cell quantity in gastric cancer patients was significantly higher （75.53 ± 2.20） % than that in healthy controls （12.83 ± 0.94） % （ t = 19. 67, P 〈 0.01 ）. Conclusion The expression of ILT3 in PBMC may be associated with the differentiation stage and infiltration degree of tumor in gastric cancer patients, and the high expression of ILT3 in CD3^＋CD8^lowT cell subset may be involved in immune escape of tumorous cells.
