大动脉粥样硬化性及穿支动脉疾病脑卒中相关危险因素分析

摘　　要：目的探讨大动脉粥样硬化性脑卒中(LAA)及穿支动脉疾病(PAD)脑卒中的相关危险因素。方法选择行全脑数字减影血管造影术检查的缺血性脑卒中患者367例,根据中国缺血性卒中亚型的分型标准,分为LAA组219例、PAD组148例。测量入院时血压,询问动脉粥样硬化相关危险因素,检测2组血脂、空腹血糖、糖化血红蛋白、纤维蛋白原、同型半胱氨酸水平;分析2组相关危险因素。结果与PAD组比较,LAA组年龄、糖尿病、纤维蛋白原、空腹血糖、糖化血红蛋白明显升高,HDL/LDL比值明显降低(P<0.05,P<0.01),LAA组血压虽有降低趋势,但差异无统计学意义(P>0.05)。结论脑卒中预防应严格控制动脉粥样硬化的危险因素,控制患者血糖及纤维蛋白原水平,有助于减少LAA脑卒中的发生率。Objective To study the relation between ischemic stroke due to large artery atheroscle- rosis（LAA） and perforating artery disease （PAD） and its risk factors. Methods Three hundred and sixty-seven patients with ischemic stroke undergoing cerebral DSA were divided into LAA group （n=219） and PAD group （n=148） according to the Chinese ischemic stroke subclassifica- tion. Their blood pressure and serum levels of blood lipid,fasting blood glucose, HbAlc,fibrinogen and homocysteme were measured,and risk factors for atherosclerosis were recorded at admis- sion. Risk factors for ischemic stroke were analyzed. Results The age was older,the incidence of biabetes mellitus and the serum levels of fibrinogen, fasting blood glucose, HbAlc, were significantlt higher cnnd the HDL/LDL ratio was significantlt lower in LAA group than in PAD group （P〈0.05, P〈0.01）. No significant difference was observed in blood pressure between the two groups although it tended to decrease in LAA group （P〉0.05）. Conclusion Risk factors for arteriosclerosis shuld be strictly controlled in prevention of ischemic stroke. Control of serum blood glucose and fibrinogen levels helps to reduce the incidence of ischemic stroke due to large artery atherosclerosis.
