半椎板入路微创手术治疗椎管内肿瘤

摘　　要：目的探讨经一侧半椎板入路微创手术在切除椎管内肿瘤中的应用。方法回顾分析了采用经一侧半椎板切除微创入路,应用显微外科技术切除椎管内肿瘤27例的临床资料,其中神经鞘瘤9例,偏左侧5例,偏右侧2例,正中位2例,脊膜瘤8例,偏左侧4例,偏右侧2例,正中位2例,肠源性囊肿1例,偏右侧,上皮样囊肿1例,正中位,炎症1例,偏右侧,血管母细胞瘤1例,正中位,海绵状血管瘤1例,偏左侧,室管膜瘤4例,均位于正中位,星形细胞瘤1例;肿瘤位于颈段5例,胸段11例,胸腰段4例,腰骶段7例。结果除1例髓内星形细胞瘤行部分切除外,其余肿瘤均获全切,术后2周内即可下床活动,随访期内症状和体征均明显改善,全部病例均能恢复正常工作,无手术并发症和脊柱畸形,全切除肿瘤病例未见复发。结论经一侧半椎板切除微创入路,能够最大程度保留脊柱后部的骨结构,对脊柱后部的肌肉、韧带破坏小,有利于脊柱稳定性,具有创伤小、出血少、安全、卧床时间短等优点,利于患者尽快恢复。Objective To study the unilateral hemiaminectomy approach for the mierosurgical treatment of the spinal cord tumors. Meth ods Twenty-seven patients with spinal cord tumors were operated by microsurgical treatment through the unilateral hemilaminectomy at our hospital,including 9 schwannomas,8 meningiomas, 1 epidermoid cysls, 1 cavernous angiomas, 1 astrocytomas,in which 5 were cervi cal, 13 thoracic and 9 lumbar. Results All the tumors were totally excised except 1 astrocytomas. The patients who were operated could move after two weeks. The symptoms and signs were obviously improved without complication of surgery and spinal abnormality. Conclu sions Microsurgery through unilateral hemilaminectomy which can hold the muscle,ligament and bone structure of post-spine at most, profits the stability of spinal and has other merits, including slight surgery trauma, safety and short time of bed rest. So this method is worth promoting and applying.
